Message Board Disclaimer It is now apparent that patients with psoriasis are prone to a variety of other disease conditions, so-called comorbidities. Cardiovascular disease, diabetes, hypertension, inflammatory bowel disease, hyperlipidemia, liver problems, and arthritis are more common in patients with psoriasis. It is very important for all patients with psoriasis to be carefully monitored by their primary care providers for these associated illnesses. The joint inflammation of psoriatic arthritis and its complications are frequently managed by rheumatologists. This treatment uses natural or artificial ultraviolet light. The simplest and easiest form of phototherapy involves exposing your skin to controlled amounts of natural sunlight. Different systemic medicines may be tried at different times and the risks as well as benefits should be considered. Cyclosporine and methotrexate are two non-biological systemic treatments for psoriasis (see also the overview on systemic treatments). Both medicines work in different ways by slowing down the skin cell production and reducing inflammation. They are usually taken as tablets, but can also be given as injections. Topical treatments, such as moisturizers, over-the-counter and prescriptions creams and shampoos, typically are used for mild psoriasis. figure icon Register Here Bulimia Nervosa (young people) (Young People) HIT Daudén E *—Other potential side effects associated with corticosteroids include folliculitis, telangiectasia, hypertrichosis, acneiform eruptions, hypopigmentation, perioral dermatitis, striae, miliaria, glycosuria, hyperglycuria and manifestations of Cushing's syndrome. The presence of hair not only impacts the application and penetration of medications to affected areas, but also strongly influences treatment adherence. Patients commonly complain of the greasy effect of medications in this area, and difficulty removing products from their hair. Currently, topical medications are recommended as a first line of treatment in mild-to-moderate psoriasis but can also be used concomitantly with phototherapy, systemic or biological therapies for moderate-to-severe psoriasis. Topical corticosteroids, with or without the addition of the vitamin D analog calcipotriol, are the gold standard for the treatment of scalp psoriasis. Importantly, in recent years, there have been a number of new formulations introduced to the market (eg, foams, shampoos, and sprays) that enhance cosmetic acceptability and adherence. In severe or recalcitrant cases, systemic treatment should be considered. For this reason, most insurance plans do not do blanket approval of any and all FDA-approved medications and will often require a staged approval process, where a patient will have to have been unresponsive or have had significant adverse effects to less expensive medications before more expensive treatments are considered. This is even more problematic when there are attempts to do off-label psoriasis treatment using medications indicated for other inflammatory and arthritic conditions. Such use, even if supported by the scientific literature, is often be branded "experimental", and insurance coverage may be difficult or impossible to obtain. The only cutaneous side effect of calcipotriene is local irritation, which occurs in approximately 15 percent of patients.10 Calcipotriene is not recommended for use on the face or with occlusion. Hypercalcemia is a potential side effect of this agent when the dosage exceeds 100 g per week. This effect does not usually occur with weekly use of 100 g or less.11,12 Most reports of hypercalcemia have been in patients who received prolonged therapy with 200 g or more per week. For localized psoriasis, the recommended dosages do not require monitoring of serum or urinary calcium levels. However, calcipotriene should be used with caution in patients with compromised renal function or a history of renal calculi.
