Psoriasis has a strong genetic element (it runs in families). The risk of inheriting the condition is greatest in children with both parents and a sibling affected, and children with one or both parents (but no sibling) affected are also at increased risk. Age of onset is also influenced genetically, so people whose parents developed the condition early in life are also likely to develop psoriasis early in life. Studies have identified several genes which may affect the risk of a person developing psoriasis.
Journal of Membrane Science & Technology Open Access Journal Methotrexate (MTX), cyclosporine, and acitretin are the most commonly prescribed systemic medications for severe psoriasis in the United States.5 Despite the risk for hepatotoxicity, MTX appears to have the best combined safety and efficacy profile in terms of serious adverse events compared to other systemic agents.11 Guidelines for MTX monitoring, especially with regard to when to do a liver biopsy, have been substantially liberalized over time, and the recommended interval for biopsy has been extended by years; biopsy was previously recommended after a cumulative MTX dose of 1 to 1.5 g, but guidelines now suggest biopsy after 3.5 to 4 g in low-risk patients.5 While abnormally elevated liver function tests during treatment with MTX may necessitate liver biopsy, the use of transient elastography and a panel of serum biomarkers for liver function also can be used to monitor noninvasively for hepatotoxicity before biopsy is considered; these recommendations are likely to be incorporated into newer guidelines in development.12 Methotrexate has demonstrated safety and increased efficacy when used in combination with biologic agents such as adalimumab, etanercept, infliximab, and secukinumab7 and has been studied in combination with many biologics indicated for PsA.13

Do you have any other chronic conditions?—and examine the affected area. Your medical and family history, in conjunction with your physical exam, can help your dermatologist develop the most accurate diagnosis.

How could I have gotten psoriasis? What can reduce psoriasis flare-ups? (video) Home remedies Doctors aren't sure why people get psoriasis, but they do know how the disease works. We all have a kind of white blood cell, called a T lymphocyte (or T cell), in our blood. These cells are part of the immune system. They travel through the bloodstream fighting off bacteria, viruses, and other things that make us sick. Psoriasis causes a person's T cells to mistakenly attack healthy skin as if they were trying to fight an infection or heal a wound.

Psoralens make the skin more sensitive to long-wave UVA (320-400 nm). Methoxsalen (Oxsoralen) is a psoralen that is taken by mouth before UV-A light therapy. More than 85% of patients report relief of disease symptoms with 20-30 treatments. Therapy is usually given two to three times per week on an outpatient basis, with maintenance treatments every two to four weeks to maintain remission. Adverse effects of PUVA therapy include nausea, itching, and burning. Long-term complications include increased risks of sensitivity to the sun, sunburn, skin cancer, skin aging and cataracts. Protective glasses must be worn during and after treatment to prevent cataracts. PUVA therapy is not used for children younger than 12 years of age.
