Skin from a biopsy will show clubbed epidermal projections that interdigitate with dermis on microscopy. Epidermal thickening is another characteristic histologic finding of psoriasis lesions. The stratum granulosum layer of the epidermis is often missing or significantly decreased in psoriatic lesions; the skin cells from the most superficial layer of skin are also abnormal as they never fully mature. Unlike their mature counterparts, these superficial cells keep their nucleus. Inflammatory infiltrates can typically be visualized on microscopy when examining skin tissue or joint tissue affected by psoriasis. Epidermal skin tissue affected by psoriatic inflammation often has many CD8+ T cells while a predominance of CD4+ T cells makes up the inflammatory infiltrates of the dermal layer of skin and the joints.

Health Problems Another sub-analysis of the PSUNRISE trial, a trial evaluating the effect of infliximab in patients who failed etanercept, was done looking at disease activity within four designated body regions (scalp, face, nails, and palms/soles) as assessed by investigators using a 10 cm visual analog scale at weeks 1, 10, and 26. Improvement was defined as a minimum of 1.1 cm reduction from baseline in patients who had clinically active disease in all areas. The regional response rate was highest for the scalp at week 10, compared to other areas examined.52

Dr. Tan Wei Hsia Audrey The Biologics: Adalimumab (Humira), etanercept (Enbrel), infliximab (Remicade), ustekinumab (Stelara), secukinumab (Cosentyx), and ixekizumab (Taltz) are proteins, also called "biologics," that are produced by microorganisms and work on the immune system by blocking certain specific chemical messengers of the inflammatory pathway. For example, tumor necrosis factor (TNF) is involved in inflammation and is blocked by three of these medications. They all are expensive.
Tar-Containing Preparations Though some home remedies can be helpful, others could interact with medications. A person thinking about using home remedies should talk to a doctor first. Psoriasis symptoms should always be tracked to ensure they are not getting worse.

The use of biologic agents (proteins with pharmacologic activity) is discussed in Section 1 (2008) and reviewed, with updated safety information, in Section 6 of the AAD guidelines. The AAD recommends a set of baseline laboratory studies before starting treatment with a biologic agent to ensure any underlying conditions or risk factors are understood. [35, 38] Some patients with chronic hepatitis C may be safely treated with biologic agents, while active hepatitis B is still considered a contraindication.

Obesity, especially abdominal obesity, is considered a low-grade pro-inflammatory state, which increases the risk of cardiovascular disease and metabolic syndrome. Recently, several studies have been documenting a clear association between obesity and psoriasis. Relation between body mass index (BMI) and severity and extent of psoriasis lesions (PASI) appears to be direct and bidirectional.56-57
