One important step to achieve this goal is to assess the preferences that individuals have for the attributes of their therapy. Preferences is an economic concept referring to the value that an individual attaches to different options when presented with a choice. More commonly acknowledged in other fields, the role and potential impact of patients' preferences is being increasingly recognized in the health care sector.19 Previous studies20,21 in psoriasis have used time trade-off or standard gamble preference elicitation methods in which participants are asked their willingness to trade off future life years or accept a risk of immediate death in exchange for returning to full health. Studies also have assessed health state utilities in psoriasis and psoriatic arthritis using willingness-to-pay analyses, visual analog scales,20,22,23 the EuroQol-5 multiattribute health utility instrument,24 and, recently, conjoint analysis (CA).25-27

Immune: The immune system is made up of cells that help the body fight infection. With psoriasis, the immune system becomes overactive causing these immune cells to move to the top layer of skin (the epidermis) and act as if they are healing a wound. They stimulate skin cells to grow too quickly, and this causes the thick, scaly build-up seen in psoriasis plaques. During this process, these hyperactive immune cells cause your blood cells to dilate or widen (making the patches very red) and more immune cells move towards the surface of the skin. The result is a cycle in which new cells are made too quickly, in days instead of weeks. The dead skin can’t fall off fast enough, which leads to plaque formation. This process only stops when treatment interrupts the cycle.

Several of these drugs are approved for the treatment of moderate to severe psoriasis. Most of these drugs are given by injection (apremilast is oral) and are usually used for people who have failed to respond to traditional therapy or who have associated psoriatic arthritis. In particular, people taking these treatments must be screened for tuberculosis.

Psoriasis starts as small red bumps that grow in size, on top of which scale forms. These surface scales shed easily, but scales below them stick together. When scratched, the lower scales may tear away from the skin, causing pinpoint bleeding. As the rash grows larger, “plaque” lesions can form.
