Itching is one of the most common–and vexing–symptoms of scalp psoriasis, says Harold Farber, MD of Philadelphia. And itching begets inflammation and can cause your scalp psoriasis to spread to previously unaffected areas via the Koebner phenomenon. Scalp psoriasis tends to itch more than other body parts, and not scratching is easier said than done, Dr. Farber says. Making matters even more difficult, stress tends to travel with psoriasis and many of us cope with stress by … scratching. Psoriasis often has a severe psychological impact and 10% of affected individuals between 18–34 years of age report that they have contemplated suicide. The vast majority report less serious psychological complaints including concern for the future, embarrassment, negative self-image and depressed feelings. Young people (18–34 years old) experience psychological complaints most often, with 81% reporting feeling embarrassed when people saw their skin rash and 75% reporting they felt ugly. Abnormal production of skin cells (especially during wound repair) and an overabundance of skin cells result from the sequence of pathological events in psoriasis. Skin cells are replaced every 3–5 days in psoriasis rather than the usual 28–30 days. These changes are believed to stem from the premature maturation of keratinocytes induced by an inflammatory cascade in the dermis involving dendritic cells, macrophages, and T cells (three subtypes of white blood cells). These immune cells move from the dermis to the epidermis and secrete inflammatory chemical signals (cytokines) such as interleukin-36γ, tumor necrosis factor-α, interleukin-1β, interleukin-6, and interleukin-22. These secreted inflammatory signals are believed to stimulate keratinocytes to proliferate. One hypothesis is that psoriasis involves a defect in regulatory T cells, and in the regulatory cytokine interleukin-10.

Psoriasis is an inflammatory disease of the skin that is estimated to affect about 2.2% of the adult population. In children, the onset of psoriasis can be before the age of one year but peaks around 5-8 years. Psoriasis produces scaly, itching bumps on the skin. Some people may have a genetic predisposition to psoriasis. The genes affected seem to be involved with control of the immune system. Psoriasis appears as red scaling, slightly raised bumps (papules) that merge to form plaques. Psoriasis classically appears on the elbows and knees, but it can affect any part of the skin. The scalp is also characteristically affected in many people. Like psoriasis anywhere on the body, scalp plaques produce excess scale and can itch. Severe disease can cause a loss of scalp hair, which usually will return if the disease can be controlled. Scalp psoriasis somewhat difficult to treat when the scalp is covered with hair sufficient to act a barrier to the application of topical medications.
