Side effects of topical corticosteroids, especially those that carry the superpotent categorisation, include cutaneous atrophy, development of striae, formation of telangiectasia, and a host of other local cutaneous effects such as the formation of an acneiform eruption known as perioral dermatitis on the face.6,7 Hypothalamic–pituitary–adrenal (HPA) axis suppression can occur with prolonged use of excessive quantities of topical corticosteroids, particularly if they are occluded or if superpotent corticosteroids are used continuously over large areas of the body. However, the cutaneous side effects are more commonly problematic than significant HPA axis suppression, which is seldom an issue.8

Hypercalcemia is a potential side effect of this agent when the dosage exceeds 100 g per week. This effect does not usually occur with weekly use of 100 g or less.11,12 Most reports of hypercalcemia have been in patients who received prolonged therapy with 200 g or more per week. For localized psoriasis, the recommended dosages do not require monitoring of serum or urinary calcium levels. Disease that is considered too extensive to be treated by topical approaches, that is usually greater than 5%-10% of the total body surface area, is an appropriate indication for this sort of treatment. Resistance to conventional topical treatment is another indication for light therapy. Although normal sunlight contains these wavelengths, self-exposure to sunlight must be done in under controlled conditions to minimize burns. In a physician's office, control of the amount of light energy administered to each patient is essential. Medical light sources use special wavelengths of light and timers to assure the correct dosage of light. Sunlamps and tanning booths are not acceptable substitutes for medical light sources. Ultraviolet light from any source is known to produce skin cancer, but this side effect is minimized when the light is appropriately administered in a physician's office.

Individuals with psoriasis aged 18 years or older who attended outpatient clinics at the Dermatology Department of the University Medical Centre Mannheim between December 1, 2009, and September 30, 2010, were asked to participate in the study. To ensure that the full range of antipsoriatic treatments (ie, local therapy, UV therapy, classic systemic therapy, or biologicals) would be valid options, only patients with moderate to severe psoriasis according to the criteria of the Committee for Medicinal Products for Human Use29 were recruited. These included (1) those with a Psoriasis Area and Severity Index (PASI) of 10 or more30; (2) patients with psoriatic involvement of the hands, feet, or head; (3) patients with psoriatic arthritis according to Classification of Psoriatic Arthritis criteria31; and (4) those receiving systemic antipsoriatic therapy. Patients visiting our department for the first time, patients coming for follow-up visits, and those with and without ongoing antipsoriatic therapy were included. The study was approved by the Ethics Committee of the Medical Faculty Mannheim and performed according to the principles of the Declaration of Helsinki.
