P It is generally accepted that scalp psoriasis, like all psoriasis, is related to genetic defects that affect certain parts of the immune system. There are undoubtedly environmental risk factors that trigger its initial development in genetically predisposed individuals. The notion that "emotional stress" plays a causal role or at least exacerbates psoriasis has been difficult to prove. There is no question, however, that psoriasis of the scalp can be an extremely stressful experience.

Both the hair and any scale covering the disease act as an impediment to treatment. Removal of the scale in a nontraumatic fashion is very important. This can be accomplished by shampooing frequently. Using tar, selenium, or salicylic acid-containing shampoos can be helpful. This may have to be done at least twice a day initially if the scale is sufficiently thick. Gently rubbing off the scale with the fingertips and not the nails is important.

The rate of psoriasis varies according to age, region and ethnicity; a combination of environmental and genetic factors is thought to be responsible for these differences. It can occur at any age, although it most commonly appears for the first time between the ages of 15 and 25 years. Approximately one third of people with psoriasis report being diagnosed before age 20. Psoriasis affects both sexes equally.

In a recent multi-center UK audit on the treatment of moderate to severe pediatric psoriasis, refractory to topical therapy and/or phototherapy, methotrexate appears as the most prescribed agent, followed by acitretin and finally cyclosporine. Combining the data from this study with the literature review, the tolerance and safety of these drugs seem reasonable; thus, more comparative studies are needed to guide the clinician in decision making, considering their efficacy and safety.52

Vitamin D analogues are used to help control overactive skin cell production by binding to the vitamin D receptors on the skin cells. Topical vitamin D analogues such as calcipotriene (Dovonex, Sorilux) and calcitriol (Vectical) are effective at slowing the growth of the skin cells and can be used with emollients and topical corticosteroids applied to the skin. Calcipotriene with betamethasone (Taclonex, Enstilar) is a vitamin D analogue that is already combined with a corticosteroid.
Risk of myocardial infarction in patients with psoriasis. Increasing alcohol consumption is associated with increasing disease severity in men, and both men and women who consume alcohol have an increased risk of psoriasis. Therefore, reducing alcohol consumption is an important behaviour for preventing psoriatic skin rash.
In terms of clearance, as assessed by the IGA, steroids were better than vitamin D (RR 1.82; 95% CI 1.52 to 2.18; four studies, 2180 participants, NNTB = 8; 95% CI 7 to 11; moderate quality evidence). Statistically, the two-compound combination was superior to steroid monotherapy, however the additional benefit was small (RR 1.22; 95% CI 1.08 to 1.36; four studies, 2474 participants, NNTB = 17; 95% CI 11 to 41; moderate quality evidence). The two-compound combination was more effective than vitamin D alone (RR 2.28; 95% CI 1.87 to 2.78; four studies, 2008 participants, NNTB = 6; 95% CI 5 to 7; high quality evidence).
