Other cytokines such as IL-17 and IL-22 also have been targets for inhibition as they play important roles in the pathogenesis of psoriasis. Another avenue of research has focused on the use of vascular endothelial growth factor inhibitors to treat psoriasis. Oral agents being investigated as alternatives to medications administered by injection include Janus kinase inhibitors, protein kinase C inhibitors, mitogen-activated protein kinase inhibitors, and phosphodiesterase 4 inhibitors, all of which have proven effective in various phase 2 and 3 clinical trials. However, these agents have potentially severe side-effects due to their immunosuppressive mechanisms.

Dermatologic drug shortages Disease is graded as mild, moderate, or severe based on the body surface area affected and how the lesions affect the patient's quality of life. To be considered mild, usually < 10% of the skin surface should be involved. There are many more complex scoring systems for disease severity (eg, the Psoriasis Area and Severity Index), but these systems are useful mainly in research protocols.
Permissions Podcasts Skin Problems Slideshow Meditation Psoriasis has a strong genetic element (it runs in families). The risk of inheriting the condition is greatest in children with both parents and a sibling affected, and children with one or both parents (but no sibling) affected are also at increased risk. Age of onset is also influenced genetically, so people whose parents developed the condition early in life are also likely to develop psoriasis early in life. Studies have identified several genes which may affect the risk of a person developing psoriasis.

Chandran V We opted to treat the patient with a 308-nm excimer laser (Xtrac; PhotoMedex, Radner, Pa) that has a fiber-optic handpiece connected to a scalp delivery device (PhotoMedex) (Figure 1). The scalp delivery device functions by displacing hair that is interfering with access of the laser beam to the scalp. It looks and operates like a blow-dryer but does not deliver any heat to the scalp.

Many children if young will accept their skin problems as a matter of course, whilst others, depending on their age of onset may take their conditions differently and feel embarrassed, upset, angry, stressed or even depressed. They may feel anxious about recurring flare-ups once they’ve experienced good periods of remission, and become pre-occupied and distracted from normal daily activities. As they grow up, possibly pre-occupied with their body, body image and peer pressure, their psoriasis may become more of an issue for them. Love, support, encouragement and trust in their medical team too will help overcome such stressful periods in their lives. Parents should always be understanding and aware of such issues especially if their child has psoriasis and/or psoriatic arthritis.

Eczema/ Dermatitis PUVA is a special treatment using a photosensitizing drug and timed artificial-light exposure composed of wavelengths of ultraviolet light in the UVA spectrum. The photosensitizing drug in PUVA is called psoralen. Both the psoralen and the UVA light must be administered within one hour of each other for a response to occur. These treatments are usually given in a physician's office two to three times per week. Several weeks of PUVA is usually required before seeing significant results. The light exposure time is gradually increased during each subsequent treatment. Psoralens may be given orally as a pill or topically as a bath or lotion. After a short incubation period, the skin is exposed to a special wavelength of ultraviolet light called UVA. Patients using PUVA are generally sun sensitive and must avoid sun exposure for a period of time after PUVA. Common side effects with PUVA include burning, aging of the skin, increased brown spots called lentigines, and an increased risk of skin cancer, including melanoma. The relative increase in skin cancer risk with PUVA treatment is controversial. PUVA treatments need to be closely monitored by a physician and discontinued when a maximum number of treatments have been reached.
