They should be applied liberally and frequently. Treatment considerations Psoriasis can be classified into four types: plaque-type psoriasis, guttate psoriasis, localized pustular psoriasis and generalized pustular psoriasis. (Table 1). The less common forms of psoriasis include pustular (localized and generalized) and erythrodermic variants. The most common form is plaque-type psoriasis (Figure 2). The scale itself is variable, ranging from a thick, massive scale, as is generally seen on the scalp, to no scale at all, as is generally seen in intertriginous or partially treated areas.

The physical and mental disability experienced with this disease can be comparable or in excess of that found in patients with other chronic illnesses such as cancer, arthritis, hypertension, heart disease, diabetes, and depression. A study by Kurd et al further supports the notion that psoriasis impacts quality of life and potentially long-term survival. There should be a higher clinical suspicion for depression in the patient with psoriasis.

Teenagers: epilepsy (Young People) Journal of Hotel & Business Management Open Access Journal PSORIASIS TREATMENT For Caregivers & Loved Ones MACRA Many novel medications being researched target the Th17/IL-23 axis, particularly IL-23p19 inhibitors, as IL-23p19 is present in increased concentrations in psoriasis skin lesions while contributing less to protection against opportunistic infections. Other cytokines such as IL-17 and IL-22 also have been targets for inhibition as they play important roles in the pathogenesis of psoriasis. Another avenue of research has focused on the use of vascular endothelial growth factor inhibitors to treat psoriasis. Oral agents being investigated as alternatives to medications administered by injection include Janus kinase inhibitors, protein kinase C inhibitors, mitogen-activated protein kinase inhibitors, and phosphodiesterase 4 inhibitors, all of which have proven effective in various phase 2 and 3 clinical trials. However, these agents have potentially severe side-effects due to their immunosuppressive mechanisms.
Excimer lasers or pulsed dye lasers are used in laser therapy. A pulsed dye laser will create a concentrated beam of yellow light. When this light hits the skin, it converts to heat. The heat then destroys the extra blood vessels in the skin that contribute to psoriasis. Excimer lasers will deliver ultraviolet light to localized areas of the skin that help treat psoriasis. These lasers produce UV light in wavelengths similar to narrow-band UVB. Laser therapy uses intense doses of laser light to help control areas of mild to moderate psoriasis without damaging surrounding healthy skin. These can be quite effective for small plaques of psoriasis, but since only small areas of skin can be treated at once, they are not practical for extensive disease.

Genes & Psoriatic Arthritis Abbreviations Physician Leadership Phototherapy is the use of UV radiation to treat skin disorders, and this can be very effective in the treatment of psoriasis. It is generally reserved for cases where topical therapy has been ineffective or too much of the skin surface is involved to treat psoriasis effectively with topical agents. It is administered in cabinets at specialised centres, and a treatment course for psoriasis will usually consist of 2–3 treatments per week for 20–30 treatments.

The Goeckerman regimen uses coal tar followed by UV-B exposure and has been shown to produce improvement in more than 80% of patients. The odor of coal tar limits its popularity. The treatments involve twice-a-day light exposure plus daily application of the tar preparation for two to four weeks. This is no small commitment and either requires hospitalization or using a psoriasis day-care treatment center.

During the study period, 197 outpatients with psoriasis were asked to participate. Sixteen potential participants (8.1%) declined enrollment and 18 potential participants (9.1%) were excluded because the psoriasis was classified as mild. All 163 participants completed the computerized survey. Mean (SD) age was 49.3 (14.1) years (range, 18-80 years). Further sociodemographic and socioeconomic characteristics are reported in Table 3. Despite the fact that only participants with moderate to severe psoriasis were included, the mean (SD) PASI was relatively low (5.6 [5.5]; range, 0-32.8) because most participants were receiving antipsoriatic treatment at the time of data collection. The mean (SD) DLQI was 7.6 (6.9; range, 0-29, Table 3).
