Drugs that alter the immune system (biologics). Several of these drugs are approved for the treatment of moderate to severe psoriasis. They include etanercept (Enbrel), infliximab (Remicade), adalimumab (Humira), ustekinumab (Stelara), golimumab (Simponi), apremilast (Otezla), secukinumab (Cosentyx) and ixekizumab (Taltz). Most of these drugs are given by injection (apremilast is oral) and are usually used for people who have failed to respond to traditional therapy or who have associated psoriatic arthritis. Biologics must be used with caution because they have strong effects on the immune system and may permit life-threatening infections. In particular, people taking these treatments must be screened for tuberculosis.

Chronic Plaque Psoriasis When we performed subgroup analyses, we found that preferences are, in part, dependent on sociodemographic and socioeconomic characteristics. Older patients ( ≥65 years) appear to care less about the probability of improvement compared with younger individuals. In most other studies20,21 using time trade-off, willingness to pay, and visual analog scales to assess health state utilities in psoriasis, utilities were largely independent of age. However, psoriasis-related impairment may be perceived differently among different age groups. For example, it has been reported5,12,40,41 that older patients experience psoriasis more physically than mentally compared with younger patients. Correspondingly, younger patients with psoriasis were shown22 to have greater willingness to pay for social comfort and emotional health than were older patients. Our descriptive analyses also showed the tendencies that women care more about the probability of improvement than men and that singles place greater value on this attribute compared with individuals living with a partner (Figure 2A and C). Although differences did not achieve statistical significance in our sample, it seems likely that women and singles attach special importance to appealing appearance, not the least because of expectations from society and from potential partners.

Moderate to severe plaque psoriasis should be treated with topical agents and either phototherapy or systemic agents. Immunosuppressants are used for quick, short-term control (eg, in allowing a break from other modalities) and for the most severe disease. Immunomodulatory agents are used for moderate to severe disease unresponsive to other agents.

One randomized, double-blind study of 49 patients evaluated the efficacy of twice daily calcipotriol solution (50 mcg/mL) over a 4-week treatment period. The study reported that 60% of patients treated with calcipotriol solution were clear or demonstrated marked improvement in their symptoms as compared with only 17% in the placebo group. The total sign score for scalp psoriasis (ie, sum of the scores for redness, induration, and scale) decreased by 49% in the treatment group compared with 19% in the placebo group (P=0.005). Patients’ self-reports also demonstrated that calcipotriol was superior in reducing scalp flaking and itch.37
Though the data isn’t complete, research suggests cases of psoriasis are becoming more common. Whether that’s because people are developing the skin condition or doctors are just getting better at diagnosing it is unclear.
