Psoriasis treatments generally fall into one of three categories, including: People are curious when they see something different. You have many choices about how to react to their curiosity. You may choose to ignore their attention, or you can explain that psoriasis is a skin condition that is not contagious.
Digital Editions Health topics Figure 2. Treatment preferences among sociodemographic subgroups. Differences between relative importance scores (RISs) obtained for sociodemographic subgroups were tested for statistical significance using analysis of variance (for dichotomous variables) or post hoc tests (for >2 groups). A, No significant differences were found with respect to sex. B, When stratifying by age categories, the probability of benefit was significantly less important for participants 65 years or older (mean [SEM] RIS, 13.74 [2.25]) than for those aged 18 to 35 years (28.40 [4.50]) and those aged 50 to 64 years (27.11 [3.84]). C, Preferences of individuals living with a partner were similar to those of singles. * P = .05. AE indicates adverse effect.
Most people with psoriasis experience nothing more than mild skin lesions that can be treated effectively with topical therapies.
FOOD Methotrexate-induced liver cirrhosis. A follow-up. Keratoconjunctivitis sicca can be treated with ocular lubricants and punctal occlusion. Trichiasis and cicatricial ectropion usually require surgical treatment. Conjunctival, corneal, and anterior chamber inflammation can be treated with topical corticosteroids. Nonsteroidal anti-inflammatory agents or oral corticosteroids are occasionally necessary. Whether systemic immunosuppression is effective for ocular disease is not clear. Corneal melting, inflammation, and vascularization can be difficult to treat. A bandage contact lens may retard the melting. Topical corticosteroids can control the infiltration and delay the vascularization. In some cases, progression can occur in spite of these treatments and can lead to the need for lamellar or penetrating keratoplasty.

Psoralen is a photosensitizer that is ingested prior to light exposure. PUVA treatment results in conjunctival hyperemia and dry eye, particularly if sun protection is not used. With proper eye protection, there does not appear to be a risk of cataract. Psoralens for either topical (bath) or systemic use may occasionally be difficult to obtain because of intermittent availability issues.

Ciclosporin, the most effective oral therapy for psoriasis, was fortuitously discovered to help psoriasis when it was administered to transplant patients with the disease. The side effects of ciclosporin are numerous and include hypertension, nephrotoxicity, hypomagnesaemia, hyperkalaemia, hyperuricaemia, elevation of liver function tests, development of paraesthesias, and hypertrichosis. In transplant patients, this immunosuppressive drug is associated with an increase in infections and lymphoproliferative diseases. If given for extended periods of time, nephrotoxicity occurs in most patients; hence, guidelines for the treatment of psoriasis with ciclosporin in the USA have suggested that it should not be administered for more than one year.33 All of the safety and efficacy data on the use of ciclosporin for psoriasis are based on doses up to 5 mg/kg daily, but the US Food and Drug Administration lists 4 mg/kg daily in the package insert. Nevertheless, it is usual to use higher doses for more severely affected patients.

Subgroup analysis was performed on the basis of sociodemographic factors (age, 18-35, 36-49, 50-64, or ≥65 years), sex (male or female), marital status (married or living with a partner, single, or widowed), socioeconomic factors (net monthly household income < €1000 [US $1437], €1000- €2000, or > €2000) and working status (employed or unemployed [including homemakers and retirees]), and PASI (0-5, 5.1-10, or >10) and DLQI (0-5, 6-10, 11-20, or >20). For dichotomous variables, the RIS differences between subgroups were tested for significance using analysis of variance. If there were more than 2 groups, a post hoc test was applied. Significance was assessed at P ≤ .05.

In terms of treatment response, as assessed by the PGA, steroids were more effective than vitamin D (RR 1.48; 95% CI 1.28 to 1.72; three studies, 1827 participants; NNTB = 5; 95% CI 5 to 7; moderate quality evidence). Statistically, the two-compound combination was better than steroid monotherapy, however the benefit was not clinically important (RR 1.13; 95% CI 1.06 to 1.20; two studies, 2226 participants; NNTB = 13; 95% CI 9 to 26; high quality evidence). The two-compound combination was more effective than vitamin D (RR 1.76; 95% CI 1.46 to 2.12; four studies, 2222 participants; NNTB = 4; 95% CI 3 to 6; moderate quality evidence).
