Physics & Materials Science Two major immune system genes under investigation are interleukin-12 subunit beta (IL12B) on chromosome 5q, which expresses interleukin-12B; and IL23R on chromosome 1p, which expresses the interleukin-23 receptor, and is involved in T cell differentiation. Interleukin-23 receptor and IL12B have both been strongly linked with psoriasis.[32] T cells are involved in the inflammatory process that leads to psoriasis.[30] These genes are on the pathway that upregulate tumor necrosis factor-α and nuclear factor κB, two genes involved in inflammation.[30] Recently, the first gene directly linked to psoriasis has been identified. A rare mutation in the gene encoding for the CARD14 protein plus an environmental trigger was enough to cause plaque psoriasis (the most common form of psoriasis).[33][34] Typically, a patient will have psoriasis months or years before they develop psoriatic arthritis. Psoriatic arthritis usually involves the knees, ankles, and joints in the feet. There may also be a loss of range of motion of the involved joints as well as joint stiffness. Psoriatic arthritis can also cause inflammation of the spine and the sacrum, which causes pain and stiffness in the low back, buttocks, neck, and upper back. UK Evidence of psoriasis Treat sudden flares of psoriasis on the hands and feet promptly and carefully. In some cases, cracking, blisters and swelling accompany flares. Nail changes occur in up to 50 percent of people with psoriasis and at least 80 percent of people with psoriatic arthritis. Other non-biological medications With the methotrexate cos I was given loads of information about it cos it’s such a strong drug, when I first started. And, with psoriasis it’s used in very low doses. But in higher doses it can be used for chemotherapy treatments and things like that. Which scared me a bit. But then I spoke to my dermatologist and he said, you know, “It’s in very low doses and because it can affect your liver.” I had to have blood tests every two weeks. So they were very, very good at monitoring the effect on me. But, I was in my final year of university and had final exams and essays and stuff. And, it made me feel sickly. I took it once a week. And I think I took it on a Monday and the Monday, Tuesday, Wednesday, I always felt really, really sickly and headache-y as well, which was strange. But things like going out for a meal. I’d go out for a meal with my boyfriend. I can remember one particular time, we went to this lovely Indian and I couldn’t eat anything. I had this sort of starter and I felt really sick and I love Indian food. And, so just, it affected me like that. And it was, it was a shame, really, cos my skin, I felt the best I felt, like I felt like I looked normal, you know what I mean? And but yet, I was feeling so sick and headache-y and quite tired, I think. And it just, it got to the point, I was on it for a good few months, and it got to the point where I thought, ‘I’d rather have a bit, a bit of psoriasis and keep it under control with topical things maybe, than feel like this all the time’. And like I say, it affected my social life as well. And I know it shouldn’t. It really shouldn’t have, cos you do not need alcohol to be sociable. But I think it’s [coughs] it’s quite a big aspect of uni life and things. And I did still go out and not drink and had a nice time. But I felt that my friends were thinking, ‘oh, she’s been boring, she’s not drinking’ and didn’t really understand how serious the medicine was, where you can’t drink on it, cos it can really affect your liver. So, it did come with, with a lot of problems in my experience. And unless it got very, very, very bad, I probably wouldn’t like to go on it again.  Psoriasis is an autoimmune disorder where rapid skin cell reproduction results in raised, red and scaly patches of skin. It is not contagious. It most commonly affects the skin on the elbows, knees, and scalp, though it can appear anywhere on the body. Although psoriatic plaques can be limited to only a few small areas, the condition can involve widespread areas of skin anywhere on the body. Psoriasis symptoms vary depending on the type of psoriasis you have. Common psoriasis symptoms can include the following: De Berker DA, Lawrence CM. This could be because they have scratched the lesions and made them sore, the medication has caused some irritation, even down to the weather temperatures. Cold weather can make the skin really sore and so anything against the skin may irritate and cause pain. Perhaps slightly bigger shoes for periods when their feet hurt may allow some comfort.  In the summer the heat may make their feet sweat, this again can cause some discomfort, so perhaps keeping them as dry as possible with some talcum powder may prove useful. Studies have shown that people with psoriasis have a lower quality of life and lower self-esteem than people who do not have the disorder. The physical impacts of psoriasis include irritation, pain, or burning sensations. The emotional impacts include an increased chance of depression, and impaired coping skills. People with psoriasis can feel stigmatized which may result in avoidance of social settings and increased isolation. In general, the degree of physical and emotional impairment for people with psoriasis is similar to those who have heart disease, cancer, or diabetes. According to the AAD guidelines, PUVA can result in long remissions, but long-term use of PUVA in Caucasians may increase the risk of squamous cell carcinoma (SCC) and possibly malignant melanoma. [35, 37] A prospective study of 1380 patients found a strong correlation between number of PUVA treatments and risk of developing one or more SCC. According to the study, exposure to more than 350 PUVA treatments greatly increases the risk of SCC. [40] plaque psoriasis plaque psoriasis 80 plaque psoriasis and alcohol
