Methods: The biologics compared were adalimumab, etanercept, infliximab, ustekinumab, and secukinumab. Average wholesale prices of biologics were obtained through Lexicomp. Home phototherapy costs were estimated by obtaining quotes from phototherapy device manufacturers. Three-year cost horizon, 3-month cost, and cost per success were calculated. To assess cost-effectiveness, Psoriasis Area Severity Index (PASI) 75 rates served as the surrogate for the rate of treatment success. So all of the side-, all of the major side-effects are basically about your immune system being low and you being more likely to get like serious illnesses. But the ones that I experienced were just, I had bad headaches and I felt nauseous and certain like food would make me sick. So like diet coke would make me feel really ill and things like that. Which was strange, like that wasn't written down anywhere that I could read. I just noticed it, that when I wasn't on it I could drink it and I felt fine and when I was on it I, like yeah I felt weird. And it would just make my stomach like in pain, I'd get cramps all the time. So yeah, I-, and also it affected my mood like a lot. And that was something that I wasn't that aware of, but my friends and family would tell me a lot. So I became more irritable and I also became a lot more depressed and angry and, you know, sort of emotional and crying all the time about things. And also aggressive, you know I pushed people away a lot of the time and even when the cyclosporine was helping my skin, I could feel like it was still really affecting my mood. Cos usually when my skin's better I feel like a lot calmer, [laughs] I’m a lot nicer to people when my skin's better. But with cyclosporine, I struggled with my personality. I don't know, it felt strange, it felt like I couldn't really recognise myself. I'd look back on it now and think you know I just, ‘that was a completely different person.’ Most of the time angry, just angry all the time. And I felt like I wanted sort of-, it was weird I wanted to talk about it all the time because it was like, it consumed my life and I wasn't really going out. But at the same time when I did go out I didn't want anyone to know. So when I was at home with my friends, you know, it was kind of all I'd talk about, to the point where like my mom and dad would often say to me, you know, like “There is nothing we can do, like you’re on this medication and we can't talk about it 24/7. You know, you have to just like try and cope, and try and see the bigger picture.” And I think everything would anger me, like every time somebody told me to, “See the bigger picture. You’re on medication. You are gonna be fine. You're not dying of anything.” Everything would just kind of anger me and I'd just sort of lash out. And I feel a lot calmer on this medication, it seems strange to think that medication could affect your personality so much, but it did. And I instantly sort of saw the difference when I came off it and I went on other things, even though my skin was still bad I felt different.

Several objective measures exist to help quantify the severity of scalp psoriasis. For general plaque psoriasis, common tools include the Psoriasis Area and Severity Index (PASI), the Physician Global Assessment (PGA) or Body Surface Area (BSA) scores.10,11 The most commonly employed tools for scalp psoriasis are modifications of the PASI: the Psoriasis Scalp Severity Index (PSSI), the Scalp-modified PASI, and the scalp PGA (ScPGA). Unlike the PASI, which evaluates disease severity on four distinct body surface areas, these indexes take into account erythema, induration, and desquamation of disease affecting the scalp only. Other instruments exist but are less commonly employed.11,12 There are also patient self-assessment tools that have been developed, including the Dermatology Life Quality Index as well as the Scalpdex, a quality-of-life instrument that addresses symptoms as well as disease impact.12–14 Together, these tools can be used not only to assess symptomatology, but also guide treatment choices and evaluate patient response on a variety of indicators.

Psoriasis of the scalp is a common yet difficult condition to treat. Overlying dense hair, inaccessibility to UV exposure, and noncompliance with treatment that often involves messy, malodorous topical medications are factors that frequently limit therapeutic success. Psoriasis of the scalp often brings patients to dermatologists because of itching, scaling, hair loss, and bleeding.1 Topical medications, such as tar shampoos, steroid solutions, and oils, have been the mainstays of the treatment of scalp psoriasis. Unfortunately, some patients simply do not respond to this form of therapy, and compliance requires a lot of time and motivation. Aside from topical medications, few treatment modalities exist for the treatment of scalp psoriasis. Phototherapy, while excellent for body psoriasis, proves relatively ineffective for the scalp in patients without closely shaved heads, because the hair causes mechanical hindrance for light access. Even when traditional phototherapy can be used for the treatment of scalp psoriasis, noninvolved areas of the scalp and face are often inadvertently exposed. Systemic therapies that are used to control psoriatic lesions elsewhere on the body will improve scalp lesions but are rarely indicated solely for scalp psoriasis.2 It is clear that new and innovative treatment modalities are required for the treatment of scalp psoriasis.
A highest-potency topical corticosteroid is recommended. Methotrexate (Rheumatrex) or acitretin (Soriatane; a systemic retinoic acid analog) may be needed.

In severe cases, systemic medications such as retinoids (acitretin), methotrexate, cyclosporine, 6-thioguanine, azathioprine, a biologic, or hydroxyurea may be necessary for adequate control. Retinoids have been reported to cause dry eye, blepharitis, corneal opacities, cataracts, and decreased night vision. All of these may be associated with gastrointestinal intolerance, hepatic damage (acitretin, 6-thioguanine, azathioprine, methotrexate), marrow suppression (6-thioguanine, methotrexate, azathioprine, hydroxyurea) or renal damage (cyclosporine).
