Psoriasis treatment: Phototherapy The scalp is one of the most common sites to be affected by psoriasis, and sometimes it is the only area of involvement. It usually extends to, or just beyond the hairline and commonly occurs behind the ears. Scalp psoriasis may appear in the form of red, raised plaques covered in fine white flakes (similar to dandruff), or the scale may become thickened, with an appearance like cradle cap.

The rate of psoriasis varies according to age, region and ethnicity; a combination of environmental and genetic factors is thought to be responsible for these differences. It can occur at any age, although it most commonly appears for the first time between the ages of 15 and 25 years. Approximately one third of people with psoriasis report being diagnosed before age 20. Psoriasis affects both sexes equally.
Because itching and flaking can make psoriasis look and feel worse, it is important to keep skin moisturized. Despite the vast array of treatment options available for psoriasis, psoriasis of the scalp remains a difficult treatment area. The presence of hair not only impacts the application and penetration of medications to affected areas, but also strongly influences treatment adherence. Patients commonly complain of the greasy effect of medications in this area, and difficulty removing products from their hair. Currently, topical medications are recommended as a first line of treatment in mild-to-moderate psoriasis but can also be used concomitantly with phototherapy, systemic or biological therapies for moderate-to-severe psoriasis. Topical corticosteroids, with or without the addition of the vitamin D analog calcipotriol, are the gold standard for the treatment of scalp psoriasis. Importantly, in recent years, there have been a number of new formulations introduced to the market (eg, foams, shampoos, and sprays) that enhance cosmetic acceptability and adherence. In severe or recalcitrant cases, systemic treatment should be considered.

Anti-itch treatments: Although it can be extremely itchy, scratching your scalp lesions can worsen symptoms. Treatments that help reducing itchiness, like shampoos containing menthol or salicylic acid, over-the-counter or prescription steroid creams, and even ice packs can offer some relief for mild to severe psoriasis.
