Risk Factors Stack Trace: How to Prevent Psoriasis Flare-Ups Scopus (1047) Localized pustular psoriasis is a form of psoriasis that is confined to the palms of the hands and/or the soles of the feet. The palms and soles become red and scaly, with white/yellow sterile pustules (blisters of non-infectious pus). The pus consists of white blood cells. It is not an infection and is not contagious. Reddish-brown patches are present as the pustules resolve. Psoriasis affecting the palms and soles can severely limit everyday activities, for example, walking can become difficult. This can be a very painful and debilitating form of psoriasis.

Intralesional corticosteroids have been applied in practice, although specific studies evaluating the effects of this treatment regimen on scalp psoriasis are lacking. Anecdotal reports of their use exist, and in 2009, the US National Psoriasis Foundation recommended intralesional corticosteroids as second-line treatment for scalp psoriasis.5

Oregon grape, or mahonia aquifolium, is an herbal remedy that may help calm the immune response in psoriasis. Applying a cream containing at least 10 percent mahonia aquifolium may improve symptoms. It should not be taken internally.
Take daily baths. Bathing daily helps remove scales and calm inflamed skin. Add bath oil, colloidal oatmeal, Epsom salts or Dead Sea salts to the water and soak. Avoid hot water and harsh soaps, which can worsen symptoms; use lukewarm water and mild soaps that have added oils and fats. Soak about 10 minutes then gently pat dry skin.
Sun exposure in addition to topical therapy may be beneficial when multiple areas are affected with psoriasis. Patients should be encouraged to obtain natural sunlight exposure or tanning-bed light exposure for a few minutes a day, and then to slowly increase the duration of exposure as tolerated. Unaffected areas should be covered with a sunscreen, especially the face. Ultraviolet light exposure can be used judiciously to prevent flare-ups during the maintenance phase of therapy.
Psoriasis is known to have a negative impact on the quality of life of both the affected person and the individual's family members. Depending on the severity and location of outbreaks, individuals may experience significant physical discomfort and some disability. Itching and pain can interfere with basic functions, such as self-care and sleep. Participation in sporting activities, certain occupations, and caring for family members can become difficult activities for those with plaques located on their hands and feet. Plaques on the scalp can be particularly embarrassing, as flaky plaque in the hair can be mistaken for dandruff.
Warren RB Rarely, it may be of benefit to inject triamcinolone acetonide directly into psoriatic plaques. Aside from the pain involved, the benefit only lasts about six weeks at best. Ultraviolet light in wavelengths near 313 nm (narrow band UVB) is effective in psoriasis. The hair, however, can pose a barrier to effective administration unless it is removed or sparse. The excimer laser produces out laser light in these UV wavelengths also and can be of benefit if the amount of scalp involvement is limited.
