The possibility of acting on intracellular targets has also been the object of study. Inhibition of the JAK/STAT pathway has been appealing, since JAK proteins family play a central role in signal transduction between the lymphocyte cytokine receptor and STAT proteins, involved in the immune responses promoted by lymphocytes.17 Tofacitinib, a JAK1 and JAK3 inhibitor administered orally or topically, and ruxolitinib, a JAK1 and JAK2 topical inhibitor, are the best studied JAK/STAT inhibitors to date.17-19 Inhibition of the cAMP/PKA pathway is also being investigated for the treatment of psoriasis. Inhibitors of phosphodiesterase 4 increase the intracellular levels of cyclic adenosine monophosphate (cAMP), thus inhibiting the production of pro-inflammatory mediators. Apremilast, a small inhibitory molecule specific for phosphodiesterase 4, is representative of this new therapeutic class and has recently been approved for the treatment of psoriatic arthritis.20-21

Winter activities such as snow skiing or snowboarding pose a high risk of sunburn... Topical corticosteroids (steroids, such as hydrocortisone) are very useful and often the first-line treatment for limited or small areas of psoriasis. These come in many preparations, including sprays, liquid, creams, gels, ointments, and foams. Steroids come in many different strengths, including stronger ones that are used for elbows, knees, and tougher skin areas and milder ones for areas like the face, underarms, and groin. These are usually applied once or twice a day to affected skin areas. Strong steroid preparations should be limited in use. Overuse or prolonged use may cause problems, including potential permanent skin thinning and damage called atrophy.

Plaque psoriasis Journal of Cancer Science and Research Open Access Journal Individuals with psoriasis aged 18 years or older who attended outpatient clinics at the Dermatology Department of the University Medical Centre Mannheim between December 1, 2009, and September 30, 2010, were asked to participate in the study. To ensure that the full range of antipsoriatic treatments (ie, local therapy, UV therapy, classic systemic therapy, or biologicals) would be valid options, only patients with moderate to severe psoriasis according to the criteria of the Committee for Medicinal Products for Human Use29 were recruited. These included (1) those with a Psoriasis Area and Severity Index (PASI) of 10 or more30; (2) patients with psoriatic involvement of the hands, feet, or head; (3) patients with psoriatic arthritis according to Classification of Psoriatic Arthritis criteria31; and (4) those receiving systemic antipsoriatic therapy. Patients visiting our department for the first time, patients coming for follow-up visits, and those with and without ongoing antipsoriatic therapy were included. The study was approved by the Ethics Committee of the Medical Faculty Mannheim and performed according to the principles of the Declaration of Helsinki.

Cyclosporine is a potent immunosuppressive drug used for other medical uses, including organ transplantation. It may be used for severe, difficult-to-treat cases of widespread psoriasis. Improvement and results may be very rapid in onset. It may be hard to get someone off of cyclosporine without flaring their psoriasis. Because of the potential cumulative toxicity, cyclosporine should not be used for more than one to two years for most psoriasis patients. Major possible side effects include kidney and blood-pressure problems.
