SOCIETY & EDUCATION Biologics that control the body’s immune response are routinely used to treat severe psoriasis and psoriatic arthritis. These drugs, which are made from human or animal proteins, are quite effective but extremely expensive. They include adalimumab (Humira), brodalumab (Siliq) etanercept (Enbrel), guselkumab (Tremfya) ixekizumab (Taltz), secukinumab (Cosentyx), and ustekinumab (Stelara).
Moisturizers and emollients such as mineral oil, petroleum jelly, calcipotriol, and decubal (an oil-in-water emollient) were found to increase the clearance of psoriatic plaques. Some emollients have been shown to be even more effective at clearing psoriatic plaques when combined with phototherapy. However, certain emollients have no impact on psoriasis plaque clearance or may even decrease the clearance achieved with phototherapy, eg. the emollient salicylic acid is structurally similar to para-aminobenzoic acid (PABA), commonly found in sunscreen, and is known to interfere with phototherapy in psoriasis. Coconut oil, when used as an emollient in psoriasis, has been found to decrease plaque clearance with phototherapy. Medicated creams and ointments applied directly to psoriatic plaques can help reduce inflammation, remove built-up scale, reduce skin turnover, and clear affected skin of plaques. Ointment and creams containing coal tar, dithranol, corticosteroids (i.e. desoximetasone), fluocinonide, vitamin D3 analogs (for example, calcipotriol), and retinoids are routinely used. (The use of the finger tip unit may be helpful in guiding how much topical treatment to use.)
Neutralizing drug antibodies may reduce therapeutic activity in two ways: by blocking the binding site and by forming immunocomplexes.45,46 Neutralizing anti-drug antibodies associated with treatment with infliximab, adalimumab and ustekinumab were described.46 The biological effect of non-neutralizing anti-drug antibodies is not as clear, but may consist of increased clearance of the drug by the reticuloendothelial system.45 Table 2 summarizes the evidence to date of immunogenicity in the treatment of psoriasis.
More serious side effects may occur with topical corticosteroids if used in high doses for prolonged periods. You may become resistant to the helpful effects of topical corticosteroids over time, too.

The relationship between socioeconomic status and treatment preferences appears to be more complex. According to our results, participants with intermediate household incomes ( €1000- €2000 per month), mostly belonging to the working class, attach great importance to long-lasting improvement. To achieve improvement, they are more willing to risk AEs than are participants with lower household incomes ( < €1000 per month) who are likely in Germany to be supported by social benefits. Psoriasis is known to have a negative effect on individuals' professional life and employment status.5,42 Therefore, individuals who are employed in moderate-income occupations may be more likely to demonstrate preferences for treatment compatible with work constraints, ie, a treatment with long-lasting benefit that minimizes time lost from an hourly wage job. In descriptive analyses, we also observed the tendency that participants with a monthly income of more than €1000 care more about the probability of benefit than do those with an income below that level (Figure 3A). The same applies to participants who are working compared with those who are homemakers, unemployed, or retired (Figure 3B). Although differences were not significant, our findings may indicate that individuals with an occupation attach greater importance to appearance, possibly because of expectations at work and from society. Remarkably, and in accordance with previous willingness-to-pay analyses in psoriasis,21,22 income was not a predictor for the effect of individual cost on preferences. Cost to be covered by the individual was the process attribute regarded as least important, ie, participants were prepared to trade copayments for more convenient treatment modalities independent of their income.

Phototherapy in the form of sunlight has long been used for psoriasis. UVB Wavelengths of 311–313 nanometers are most effective, and special lamps have been developed for this application. The exposure time should be controlled to avoid over exposure and burning of the skin. The UVB lamps should have a timer that will turn off the lamp when the time ends. The amount of light used is determined by a person's skin type. Increased rates of cancer from treatment appear to be small. Narrow band UVB light (NBUVB) phototherapy has been demonstrated to have similar efficacy to Psoralen and ultraviolet A phototherapy (PUVA). A 2013 meta-analysis found no difference in efficacy between NB-UVB and PUVA in the treatment of psoriasis, but NB-UVB is usually more convenient.
