Psoriasis is characterized by an abnormally excessive and rapid growth of the epidermal layer of the skin. Abnormal production of skin cells (especially during wound repair) and an overabundance of skin cells result from the sequence of pathological events in psoriasis. Skin cells are replaced every 3–5 days in psoriasis rather than the usual 28–30 days. These changes are believed to stem from the premature maturation of keratinocytes induced by an inflammatory cascade in the dermis involving dendritic cells, macrophages, and T cells (three subtypes of white blood cells). These immune cells move from the dermis to the epidermis and secrete inflammatory chemical signals (cytokines) such as interleukin-36γ, tumor necrosis factor-α, interleukin-1β, interleukin-6, and interleukin-22. These secreted inflammatory signals are believed to stimulate keratinocytes to proliferate. One hypothesis is that psoriasis involves a defect in regulatory T cells, and in the regulatory cytokine interleukin-10.
In some cases, the doctor may remove a sample of skin (known as a biopsy) to examine it more closely. A biopsy can help doctors decide whether someone has psoriasis or another condition with similar symptoms.
The good news is that patients can treat some of the signs and symptoms of psoriasis with simple measures. For example, regular use of moisturizers may improve the itching and scaling. Reducing or limiting tobacco use and alcohol consumption may decrease the number of flares of psoriasis. Lifestyle changes—such as maintaining a healthy weight and being physically active—may help lessen or prevent the development of associated diseases.
Coal tar preparations are anti-inflammatory and decrease keratinocyte hyperproliferation via an unknown mechanism. Ointments or solutions are typically applied at night and washed off in the morning. Coal tar products can be used in combination with topical corticosteroids or with exposure to natural or artificial broad-band UVB light (280 to 320 nm) in slowly increasing increments (Goeckerman regimen). Shampoos should be left in for 5 to 10 min and then rinsed out.

Avoidance of triggers Dancing With The Stars: Nikki Glaser becomes first celebrity eliminated from season 27 of ABC show During part two of the premiere Since psoriasis mainly affects the skin, topical treatments are very useful because they are relatively safe, fairly effective, and can be applied directly to the affected skin. They take the form of lotions, foams, creams, ointments, gels, and shampoos. They include topical steroids, tar preparations, and calcium- modulating drugs. The precise drug used and the form in which it is delivered depends on the areas involved. In widespread disease in patients with more than 10% of the body surface involved, it may not be practical to use topical medication alone.

Guidelines regard biologics as third-line treatment for plaque psoriasis following inadequate response to topical treatment, phototherapy, and non-biologic systemic treatments. The safety of biologics during pregnancy has not been assessed. European guidelines recommend avoiding biologics if a pregnancy is planned; anti-TNF therapies such as infliximab are not recommended for use in chronic carriers of the hepatitis B virus or individuals infected with HIV.

FIND A FREE SPOTme® SKIN CANCER SCREENING Topical treatments such as creams and gels which are applied directly to the site of psoriatic skin rash are the most commonly used therapies. Topical treatments are typically safe and may be used alone (in cases of mild disease) or in combination with systemic or phototherapies (discussed below) in cases of moderate-severe disease. When deciding which topical treatment is best, it is important to consider the time and difficulty involved in applying the treatment. Preparations which can be applied easily with little mess are recommended, as when the treatment is easy to use, it is more likely to be used frequently and as recommended by the doctor.

Tregalizumab, another biotechnological agent under study in the treatment of psoriasis, which acts through the specific stimulation of a CD4 epitope, has the potential to exclusively activate regulatory T lymphocytes without activation of effector T lymphocytes, with an increase in anti-inflammatory cytokines (IL-10 and TGF-β), thus contributing to the disruption of the inflammatory cascade.15
Some young people report the onset of psoriasis following an infection, particularly strep throat. One-third to one-half of all young people with psoriasis may experience a flare-up two to six weeks after an earache, strep throat, bronchitis, tonsillitis or a respiratory infection. Areas of skin that have been injured or traumatized are occasionally the sites of psoriasis, know as the “Koebner [keb-ner] phenomenon.” However, not everyone who has psoriasis develops it at the site of an injury.
