Types of Psoriasis and Treatment Options How is psoriasis diagnosed and treated? More in Pubmed Scalp psoriasis is a common skin disorder that causes raised, reddish, scaly patches on the scalp and can extend beyond the hairline onto the forehead, the back of the neck and around the ears. It can vary from being very mild and unnoticeable, with slight fine scaling, or very severe with thick crusted plaques covering the entire scalp.
Site Navigation Tregalizumab, another biotechnological agent under study in the treatment of psoriasis, which acts through the specific stimulation of a CD4 epitope, has the potential to exclusively activate regulatory T lymphocytes without activation of effector T lymphocytes, with an increase in anti-inflammatory cytokines (IL-10 and TGF-β), thus contributing to the disruption of the inflammatory cascade.15
Central European Journal of Experimental Biology Open Access Journal Phototherapy has been a mainstay in psoriasis treatment for years, but today’s therapies are more effective and safer, says Dr. Farber. In particular, narrowband ultraviolet B (UVB) devices and excimer lasers can help clear psoriasis and allow you to wear short sleeves again, he says. Both treatments use UVB light, which reduces inflammation, slows the abnormal skin cell growth that drives psoriasis, and minimizes the plaques. The excimer laser zaps psoriasis plaques with a wand-like device that emits high-intensity beam of UVB light, and it works really well, he says. These treatments can be combined with other therapies to boost results. The downside? Both involve multiple trips to the dermatologist’s office for treatments.

» See SMS short codes for other countries There are a variety of drugs administered systemically that are useful in controlling psoriasis. As a generalization, most oral medications act by targeting portions of the immune system. The only exception currently is a drug called acitretin (Soriatane), which is structurally similar to vitamin A. Since the immune system is necessary in order to survive, systemic treatments do have a downside. Drugs like methotrexate and cyclosporine are administered orally and can affect the liver, kidney, and bone marrow. A new oral medication recently approved for treatment of psoriasis is called Otezla® (apremilast). Otezla selectively targets molecules inside immune cells and adjusts (reduces) the processes of inflammation within the cell, which in turn helps treat psoriasis. This drug appears to be considerably safer that most of its predecessors but is also quite expensive.

Rotational therapy refers to the substitution of one therapy for another after 1 to 2 yr to reduce the adverse effects caused by chronic use and to circumvent disease resistance. Sequential therapy refers to initial use of potent agents (eg, cyclosporine) to quickly gain control followed by use of agents with a better safety profile. Immunomodulatory agents achieve clearance or near clearance of lesions more often than methotrexate or NBUVB.
Part-worth utilities for each attribute level were computed using logit regression. The part-worth utilities were then scaled to sum to zero within each attribute. Attribute importance was assessed by calculating the range between the highest and the lowest part-worth utility for each attribute. To allow comparison between attributes, the relative importance score (RIS) for each attribute was calculated as a percentage by dividing each attribute's range by the sum of all attribute ranges and multiplying by 100.28 A RIS for each attribute was calculated for each participant and then averaged across all participants. To compare results between attribute groups 1 and 2, attribute importance was translated to one list by matching the relative importance of cost as assessed in both groups.

Psoriatic Arthritis Treatment Biologics, or immunomodulator drugs are approved for the treatment of moderate to severe psoriasis. These drugs are given by intravenous infusion, intramuscular injection or subcutaneous injection and are usually used for people who have failed to respond to traditional therapy or who have associated psoriatic arthritis. Biologics work by blocking interactions between certain immune system cells and particular inflammatory pathways. Although they are derived from natural sources rather than chemical ones, they must be used with caution because they have strong effects on the immune system and may permit life-threatening infections.
Recently, in a retrospective Spanish study of systemic therapies in psoriasis, adverse events of any type were less frequent with biotechnological agents than with conventional systemic therapy, with no age dependent differences. However, severe adverse events were more common in the elderly, probably due to the characteristics of this population and not to the treatment of psoriasis itself. Risk of adverse events with biotechnological therapy was the same for both groups, young adults and elderly. Since biotechnology appears to be safer than conventional therapy, there is a need for further studies to clarify its role in the geriatric population and to demystify its lack of safety. 49
