Advances in the treatment of moderate-severe psoriasis. Am J Health Sys Pharm. 2011; 68: 795-806. [Abstract] Clinical improvement was defined as any reduction in the baseline modified PASI score. Our patient experienced clinical improvement as early as treatment 8. Remarkably, a 90% improvement in her modified PASI score was noted at treatment 23 (Figure 2 and Figure 3), representing approximately 11 weeks of treatment (modified PASI score, 0). At this point, she reported significant improvement in her pruritus. Conversely, the untreated side (Figure 2 and Figure 4) remained thick, scaly, and extremely itchy. The entire procedure was extremely well tolerated. A subtle, transient, mild burning sensation occurred only when higher fluences were used. The patient discontinued treatment after her condition cleared. In the follow-up period assessments were performed every 3 weeks, and no further treatments were administered to either the treated or the untreated side of her scalp. The area that was treated with the excimer laser was still free of psoriasis after 10 weeks. Teige I Tags: Seborrheic-like psoriasis[edit] Cyclosporine. Cyclosporine (Gengraf, Neoral) suppresses the immune system and is similar to methotrexate in effectiveness, but can only be taken short-term. Like other immunosuppressant drugs, cyclosporine increases your risk of infection and other health problems, including cancer. Cyclosporine also makes you more susceptible to kidney problems and high blood pressure — the risk increases with higher dosages and long-term therapy. Non-biologic systemic treatments frequently used for psoriasis include methotrexate, ciclosporin, hydroxycarbamide, fumarates such as dimethyl fumarate, and retinoids.[81] Methotrexate and ciclosporin are drugs that suppress the immune system; retinoids are synthetic forms of vitamin A. These agents are also regarded as first-line treatments for psoriatic erythroderma.[14] Oral corticosteroids should not be used, for they can severely flare psoriasis upon their discontinuation.[82] Why it works: Salicylic acid is a key ingredient in over-the-counter anti-dandruff shampoos, acne medicines, and callus and wart removers. Because it’s a keratolytic, or peeling agent, it’s often used to treat scalp psoriasis. Salicylic acid also helps with the itching by managing the abnormal turnover of cells in the skin, causing those excess skin cells to soften and come off. In a recent vehicle-controlled, randomized, double-blind study involving 81 patients with moderate-to-severe scalp psoriasis, 85% of patients using CP spray were clear or almost clear of their disease after 4 weeks of twice daily application compared with only 13% in the vehicle control group (P<0.001).28 Another recent study involving 168 patients with moderate scalp psoriasis evaluated the efficacy of once daily CP 0.05% shampoo. This study demonstrated similar results, with 42% of patients using CP shampoo found to be clear or almost clear of their disease after 4 weeks of treatment compared with only 2% in the vehicle control group (P<0.001).21 Continued twice weekly use of the CP shampoo provided continued benefit to the treatment group, significantly prolonging the time to first relapse, and increasing the total percentage of patients without relapse.21 Importantly, both studies stated similar adverse event profiles between the treatment and vehicle arms, with no significant differences in scalp atrophy, telangiectasias, hypothalamic-pituitary-adrenal axis suppression or other adverse events. A recent Cochrane review found the most evidence for the use of CP in the treatment of scalp psoriasis, with four high-quality clinical trials demonstrating a combined 1.9-point change in participants’ psoriasis on a six-point global improvement scale (Investigators Global Assessment of Improvement).18 Who gets Psoriasis Methotrexate (brand names: Rheumatrex, Trexall, Otrexup, Rasuvo), a folic acid antagonist, comes as a weekly oral tablet or injection that suppresses the immune system to slow down skin cell turnover. Your doctor may also prescribe folic acid in addition to MTX to help lessen stomach side effects. The side effects of ciclosporin are numerous and include hypertension, nephrotoxicity, hypomagnesaemia, hyperkalaemia, hyperuricaemia, elevation of liver function tests, development of paraesthesias, and hypertrichosis. In transplant patients, this immunosuppressive drug is associated with an increase in infections and lymphoproliferative diseases. If given for extended periods of time, nephrotoxicity occurs in most patients; hence, guidelines for the treatment of psoriasis with ciclosporin in the USA have suggested that it should not be administered for more than one year.33 All of the safety and efficacy data on the use of ciclosporin for psoriasis are based on doses up to 5 mg/kg daily, but the US Food and Drug Administration lists 4 mg/kg daily in the package insert. Nevertheless, it is usual to use higher doses for more severely affected patients. new plaque psoriasis treatment fda approved new psoriasis biologic medications new psoriasis biologics
