In severe cases, systemic medications such as retinoids (acitretin), methotrexate, cyclosporine, 6-thioguanine, azathioprine, a biologic, or hydroxyurea may be necessary for adequate control. Retinoids have been reported to cause dry eye, blepharitis, corneal opacities, cataracts, and decreased night vision. All of these may be associated with gastrointestinal intolerance, hepatic damage (acitretin, 6-thioguanine, azathioprine, methotrexate), marrow suppression (6-thioguanine, methotrexate, azathioprine, hydroxyurea) or renal damage (cyclosporine).

Treating moderate to severe psoriasis usually involves a combination of treatment strategies. Besides topical treatments, your doctor your doctor may prescribe phototherapy (also known as light therapy). Your doctor may also prescribe systemic medications, including biologic drugs, especially if your psoriasis is significantly impacting your quality of life.
The best treatment that I've found is to do an oil or aloe vera-based mask once a week. It's a DIY treatment that's fun to make, smells great and totally gets rid of the redness on my scalp! What works for one person might not work for another, so keep trying therapies until you find one that’s effective for you.

Environmental factors such as smoking, sunburns, streptococcal sore throat, and alcoholism may affect psoriasis by increasing the frequency of flares. Injury to the skin has been known to trigger psoriasis. For example, a skin infection, skin inflammation, or even excessive scratching can activate psoriasis. A number of medications have been shown to aggravate psoriasis.
Choice of specific agents and combinations requires close cooperation with the patient, always keeping in mind the untoward effects of the treatments. There is no single ideal combination or sequence of agents, but treatment should be kept as simple as possible. Monotherapy is preferred, but combination therapy is the norm. First-line treatment for psoriasis includes topical corticosteroids and topical vitamin D3 analogs (either as monotherapy or in combination).
