Vitamin D analogues are used to help control overactive skin cell production by binding to the vitamin D receptors on the skin cells. Topical vitamin D analogues such as calcipotriene (Dovonex, Sorilux) and calcitriol (Vectical) are effective at slowing the growth of the skin cells and can be used with emollients and topical corticosteroids applied to the skin. Calcipotriene with betamethasone (Taclonex, Enstilar) is a vitamin D analogue that is already combined with a corticosteroid. Unlike generalised immunosuppressive drug therapies such as methotrexate, biologics target specific aspects of the immune system contributing to psoriasis.[81] These medications are generally well-tolerated and limited long-term outcome data have demonstrated biologics to be safe for long-term use in moderate to severe plaque psoriasis.[81][83] However, due to their immunosuppressive actions, biologics have been associated with a small increase in the risk for infection.[81] Stern DK, Creasey AA, Quijije J, Lebwohl MG. Ideally, after about 3 wk, an emollient should be substituted for the corticosteroid for 1 to 2 wk (as a rest period); this substitution limits corticosteroid dosage and prevents tachyphylaxis. Topical corticosteroid use can be expensive because large quantities (about 1 oz or 30 g) are needed for each application when a large body surface area is affected. Topical corticosteroids applied for long duration to large areas of the body may cause systemic effects and exacerbate psoriasis. For small, thick, localized, or recalcitrant lesions, high-potency corticosteroids are used with an occlusive dressing or flurandrenolide tape; these dressings are left on overnight and changed in the morning. Relapse after topical corticosteroids are stopped is often faster than with other agents. One of the most troubling features of topical corticosteroids is that patients develop tachyphylaxis, a phenomenon whereby medications that are highly effective initially, lose efficacy with prolonged use. To avoid tachyphylaxis and the other side effects of topical corticosteroids, regimens have been developed in which superpotent corticosteroids are applied twice daily for two weeks, after which they are applied on weekends only. Strong topical corticosteroids should also be avoided on the face and intertriginous sites, areas that are more prone to steroid side effects. The quantity of strong topical corticosteroids applied should be limited to 50 or 60 g per week, and occlusion should be avoided except on the scalp, palms, and soles. Strong corticosteroids should be avoided or used cautiously in children. In general, it was well tolerated, comparable with etanercept and ustekinumab, and associated with low immunogenicity; however, there is still a lack of results of long-term studies and experience in daily clinical practice that prove these findings.
