The objective of our study was to use CA to explore the relative importance of process and outcome attributes associated with available treatment options for psoriasis. Moreover, we wanted to assess whether individual preferences are influenced by sociodemographic and socioeconomic characteristics because factors such as age, employment, and marital status are known to influence adherence to antipsoriatic treatments.16

Guttate psoriasis, more common in children and adults younger than 30, appears as small, water-drop-shaped sores on the trunk, arms, legs and scalp. The sores are typically covered by a fine scale.
Psoriasis has a tendency to persist and recur. It can be an economic and social burden. Widespread ignorance of the public and misconceptions about the disease result in social avoidance and embarrassment in people with psoriasis. This may lead to social withdrawal and feelings of isolation, depression and defensive shyness. Sometimes the psychological suffering can be more severe than the physical suffering.

Phototherapy in the form of sunlight has long been used for psoriasis. UVB Wavelengths of 311–313 nanometers are most effective, and special lamps have been developed for this application. The exposure time should be controlled to avoid over exposure and burning of the skin. The UVB lamps should have a timer that will turn off the lamp when the time ends. The amount of light used is determined by a person's skin type. Increased rates of cancer from treatment appear to be small. Narrow band UVB light (NBUVB) phototherapy has been demonstrated to have similar efficacy to Psoralen and ultraviolet A phototherapy (PUVA). A 2013 meta-analysis found no difference in efficacy between NB-UVB and PUVA in the treatment of psoriasis, but NB-UVB is usually more convenient.

Corticosteroids have well-recognized anti-inflammatory and antiproliferative effects, which are thought to be their primary mechanism of action in psoriasis.6 Topical steroids are classified as low-, medium-, high- and super-high potency agents (Table 2). In general, treatment is initiated with a medium-strength agent, and high-potency agents are reserved for the treatment of thick chronic plaques that are refractory to weaker steroids. Low-potency agents are used on the face, on areas where the skin tends to be thinner, and on the groin and axillary areas, where natural occlusion increases the potency of a low-potency agent to the equivalent of a higher potency agent. Use of high-potency agents in these areas increases the risk of side effects and therefore should be avoided.

If topicals don’t work, you have options. Excimer laser therapy can be really effective for scalp psoriasis, says Dr. Farber. The laser directs UVB light to the psoriasis plaques where it helps reduce inflammation and scaling. “Sometimes intralesional injections of low dose steroids directly on to the resistant spot on scalp works nicely,” Dr. Farber says. Hypercalcemia is a potential side effect of this agent when the dosage exceeds 100 g per week. This effect does not usually occur with weekly use of 100 g or less.11,12 Most reports of hypercalcemia have been in patients who received prolonged therapy with 200 g or more per week. For localized psoriasis, the recommended dosages do not require monitoring of serum or urinary calcium levels. However, calcipotriene should be used with caution in patients with compromised renal function or a history of renal calculi.

National Psoriasis Foundation/USA Orchard Building Why Scratching Feels So Good Salicylic acid 5%–10% has a potent keratolytic effect. Salicylic acid is generally formulated in an ointment or mineral oil and applied for an extended period to reduce the hyperkeratotic scale characteristic of plaque psoriasis. In such cases, removal of scale can help with the penetration of adjunctive medications.5 Due to its formulation, patients frequently report difficulty removing the product from their hair; this can be circumvented by applying a clarifying shampoo to the hair prior to entering the shower to aid in removal of the product.
