Scopus (38) Light therapy (phototherapy) involves using natural or artificial ultraviolet (UV) light to treat the psoriasis symptoms. A doctor may recommend brief daily exposure to the sun, but too much sunlight can make psoriasis worse. More aggressive forms of light therapy include using controlled doses of UV light on the affected skin, laser therapy, and therapies that combine UV light with medicines and topical treatments.
If you have moderate to severe psoriasis — or if psoriasis stops responding to other treatments — your doctor may consider an oral or injected medication.

Phototherapy has been a mainstay in psoriasis treatment for years, but today’s therapies are more effective and safer, says Dr. Farber. In particular, narrowband ultraviolet B (UVB) devices and excimer lasers can help clear psoriasis and allow you to wear short sleeves again, he says. Both treatments use UVB light, which reduces inflammation, slows the abnormal skin cell growth that drives psoriasis, and minimizes the plaques. The excimer laser zaps psoriasis plaques with a wand-like device that emits high-intensity beam of UVB light, and it works really well, he says. These treatments can be combined with other therapies to boost results. The downside? Both involve multiple trips to the dermatologist’s office for treatments.

Excimer lasers share the common feature of producing photons from unstable excited gas dimers. The term excimer is derived from the union of excited and dimer. For the 308-nm excimer laser, the gas dimer is xenon chloride. Decomposition of this short-lived unstable dimer is responsible for the production of a collimated beam of photons with a wavelength of 308 nm.The prototype 308-nm excimer laser used in this case has the ability to produce 20- to 40-nanosecond trains of pulses with a repetition rate of up to 200 Hz. The mechanism of action is not entirely known, but we speculate that it may be similar to that of narrowband 311-nm UV-B phototherapy, which operates within the action spectrum for psoriasis.3 The laser has the advantage of being able to deliver light to specific areas, in a user-friendly, highly efficient manner, without exposing normal skin to potentially harmful radiation.

Psoriasis has environmental and genetic components. It is more common in adults and can run in families. What triggers it? A virus? Bacteria? Stress? Other environmental factors? “We just don’t know,” McCord says.
Psoriasis has a strong hereditary component, and many genes are associated with it, but it is unclear how those genes work together. Most of the identified genes relate to the immune system, particularly the major histocompatibility complex (MHC) and T cells. Genetic studies are valuable due to their ability to identify molecular mechanisms and pathways for further study and potential drug targets.
