Oral Care Cancer therapy shows promise for psoriasis treatment Etiology Five people we talked to had taken cyclosporine (Zara, Lucy, Hannah, Lisa, Sofia) and five people had tried methotrexate (Zara, Lucy, Hannah, Lisa, Megan). Everyone had taken their medicines in tablet form and Hannah had also tried methotrexate injections. Some people had tried a medicine several times at different points in their psoriasis treatment: Lisa took cyclosporine for 1 year, then methotrexate for 6 months, went back to cyclosporine for 1 month, stopped medication when living abroad and back to methotrexate when she returned to the UK. Lucy’s taken cyclosporine and methotrexate in the past, but doesn’t like “pumping my body full of medicine”. She prefers phototherapy as a psoriasis treatment.  Coal tar has been used as a treatment for psoriasis for several decades due to its anti-proliferative and anti-inflammatory actions, and its potent anti-pruritic properties.22 Although crude coal tar is the most effective tar available for the treatment of psoriasis, the application of crude coal tar to the scalp presents a challenge. As such, coal tar solution or liquor carbonis detergens 5%–20% is frequently recommended, formulated as a lotion, or added to a corticosteroid preparation. Of note, the use of coal tar has largely fallen out of favor to newer topical medications. Tar’s malodor, adverse effects (hair staining and drying), and carcinogenicity limit its use, although newer studies challenge these findings.23,24 The use of coal tar in cosmetic preparations has been banned in Canada and the European Union over concerns regarding carcinogenicity. As this class of drugs is fairly new, ongoing monitoring and adverse effect reporting continues and long-term safety continues to be monitored. Biologics are all comparatively expensive especially in view of the fact they none of them are curative. Recently, the FDA has attempted to address this problem by permitting the use of "biosimilar" drugs. These drugs are structurally identical to a specific biologic drug and are presumed to produce identical therapeutic responses in human beings to the original, but are produced using different methodology. Biosimilars ought to be available at some fraction of the cost of the original. If this will be an effective approach remains to be seen. The only biosimilar available currently is infliximab (Inflectra). Two other biosimilar drugs have been accepted by the FDA, an etanercept equivalent (Erelzi) and an adalimumab equivalent (Amjevita) -- but currently, neither are available.
