The disease manifests following exposure to various triggers (e.g., infection, medication). The typical lesions are sharply demarcated, erythematous, scaly, pruritic plaques, which occur most often on the extensor surfaces of the knees and elbows, but may also affect the scalp and back. Other common clinical findings include involvement of the nails (e.g., pitting or discoloration) or joints, which generally manifests with arthritis of the fingers and lower spine. As psoriasis presents with several subtypes, the size, location, and severity of the lesions vary. The diagnosis is based primarily on clinical findings, but may also be confirmed with tests (e.g., Auspitz sign) or biopsy. Mild psoriasis is treated with topical agents such as steroids, whereas moderate to severe disease requires systemic therapy (e.g., PUVA, biologics).

Since psoriasis is incurable, the selection of treatment plans must consider the long-term outlook. Treatment options depend on the extent and severity as well as the emotional response to the disease. They include topical agents (drugs applied to the skin), phototherapy (controlled exposure to ultraviolet light), and systemic agents (orally, intravenously, or percutaneously administered agents). Topical therapies also play an important role in the management of moderate-severe disease because when people use creams and gels, they do not need to use as much medicine or other forms of systemic treatment which have greater side effects and/or are less convenient to use. When used alone, creams and gels are generally not enough to control severe psoriasis.owever the majority of people with severe disease report never having used more intensive photo and systemic therapies.
