Methotrexate (MTX), cyclosporine, and acitretin are the most commonly prescribed systemic medications for severe psoriasis in the United States.5 Despite the risk for hepatotoxicity, MTX appears to have the best combined safety and efficacy profile in terms of serious adverse events compared to other systemic agents.11 Guidelines for MTX monitoring, especially with regard to when to do a liver biopsy, have been substantially liberalized over time, and the recommended interval for biopsy has been extended by years; biopsy was previously recommended after a cumulative MTX dose of 1 to 1.5 g, but guidelines now suggest biopsy after 3.5 to 4 g in low-risk patients.5 While abnormally elevated liver function tests during treatment with MTX may necessitate liver biopsy, the use of transient elastography and a panel of serum biomarkers for liver function also can be used to monitor noninvasively for hepatotoxicity before biopsy is considered; these recommendations are likely to be incorporated into newer guidelines in development.12 Methotrexate has demonstrated safety and increased efficacy when used in combination with biologic agents such as adalimumab, etanercept, infliximab, and secukinumab7 and has been studied in combination with many biologics indicated for PsA.13

Also reported is a randomized, placebo-controlled trial assessing the safety and efficacy of etanercept for scalp psoriasis over a 24-week period.49 In this study, Group A received etanercept 50 mg twice weekly for 12 weeks, followed by etanercept 50 mg once weekly for 12 weeks (n=62). Group B received placebo twice weekly for 12 weeks followed by 12 weeks of etanercept 50 mg twice weekly (n=62). The primary end point was percentage change in PSSI at week 12.49 The etanercept group (Group A) had a mean PSSI improvement at week 12 of 86.8%, compared to only 20.4% in the placebo group (Group B) (P<0.0001). By week 24, Group A had a mean PSSI improvement of 90.6%, which was not significantly different from Group B, which had 79.1% improvement subsequent to initiating etanercept therapy at week 13. Eighty-six percent of patients in Group A achieved a PSSI-75 at week 12, compared with only 11% in Group B (P<0.0001). However, at week 24, the PSSI-75 rates were similar at 86% and 72% for Group A and B, respectively. There was also improvement noted in scalp surface area involvement, with a mean percentage improvement at week 12 in Group A of 84.6% versus only 14.9% in Group B. At week 24, Group A maintained this benefit at 84.8%, while Group B showed improvement at 75.4%. Changes in overall PASI improvement were consistent with the PSSI improvements and time to improvement was similar; treatment also provided a high level of patient satisfaction.49

With the methotrexate cos I was given loads of information about it cos it’s such a strong drug, when I first started. And, with psoriasis it’s used in very low doses. But in higher doses it can be used for chemotherapy treatments and things like that. Which scared me a bit. But then I spoke to my dermatologist and he said, you know, “It’s in very low doses and because it can affect your liver.” I had to have blood tests every two weeks. So they were very, very good at monitoring the effect on me. But, I was in my final year of university and had final exams and essays and stuff. And, it made me feel sickly. I took it once a week. And I think I took it on a Monday and the Monday, Tuesday, Wednesday, I always felt really, really sickly and headache-y as well, which was strange. But things like going out for a meal. I’d go out for a meal with my boyfriend. I can remember one particular time, we went to this lovely Indian and I couldn’t eat anything. I had this sort of starter and I felt really sick and I love Indian food. And, so just, it affected me like that. And it was, it was a shame, really, cos my skin, I felt the best I felt, like I felt like I looked normal, you know what I mean? And but yet, I was feeling so sick and headache-y and quite tired, I think. And it just, it got to the point, I was on it for a good few months, and it got to the point where I thought, ‘I’d rather have a bit, a bit of psoriasis and keep it under control with topical things maybe, than feel like this all the time’. And like I say, it affected my social life as well. And I know it shouldn’t. It really shouldn’t have, cos you do not need alcohol to be sociable. But I think it’s [coughs] it’s quite a big aspect of uni life and things. And I did still go out and not drink and had a nice time. But I felt that my friends were thinking, ‘oh, she’s been boring, she’s not drinking’ and didn’t really understand how serious the medicine was, where you can’t drink on it, cos it can really affect your liver. So, it did come with, with a lot of problems in my experience. And unless it got very, very, very bad, I probably wouldn’t like to go on it again.
