Moderate to severe scalp psoriasis symptoms may include dandruff-like flaking, dry scalp, and hair loss. Individuals infected with HIV and those with chronic streptococcal infection have an increased risk of developing psoriasis. Those who already have psoriasis when they are infected with HIV or streptococci are likely to experience worsening psoriasis.
Most doctors consider topical treatments very effective for guttate psoriasis. However, people with guttate psoriasis may find applying the cream on so many individual spots very tiring, so topical treatments may be used more when the rash is confined to a smaller area.

MILD Jump to section + Most cases of psoriasis are treated with medications that are placed directly on the skin. These include: creams and ointments, dandruff shampoos, moisturizers, medicines containing vitamin D or vitamin A.
Adverse effects: Anthralin stains clothing or linens purple or brown. Use with caution if the individual has kidney disease. Care must be taken to apply this medication only to psoriasis patches and not to surrounding normal skin. Anthralin may cause skin discoloration (increased pigment) and may burn or irritate skin. Do not use on the face, neck, skin folds (back of knees or elbows), or genitals. Avoid contact with the eyes. Do not use on excessively irritated patches. This medication should only be used if the patient can comply with instructions for use.

Methotrexate taken orally is an effective treatment for severe disabling psoriasis, especially severe psoriatic arthritis or widespread erythrodermic or pustular psoriasis unresponsive to topical agents or UV light therapy (narrowband UVB [NBUVB]) or psoralen plus ultraviolet A (PUVA). Methotrexate seems to interfere with the rapid proliferation of epidermal cells. Hematologic, renal, and hepatic function should be monitored. Dosage regimens vary, so only physicians experienced in its use for psoriasis should undertake methotrexate therapy.

Alternatively, the National Psoriasis Foundation (NPF) recommended BSA as the preferred outcome measure in a recent consensus statement and concluded that an outcome of 3% or less BSA involvement or improvement in BSA of 75% or more is considered a desirable treatment response.9 Additionally, the Medicare Merit-based Incentive Payment System (MIPS) guidelines for successful systemic treatment response include at least 1 of the following: (1) physician global assessment score of 2 or lower, (2) BSA involvement of less than 3%, (3) PASI score lower than 3, or (4) DLQI score of 5 or lower.10
Classic genome-wide linkage analysis has identified nine loci on different chromosomes associated with psoriasis. They are called psoriasis susceptibility 1 through 9 (PSORS1 through PSORS9). Within those loci are genes on pathways that lead to inflammation. Certain variations (mutations) of those genes are commonly found in psoriasis. Genome-wide association scans have identified other genes that are altered to characteristic variants in psoriasis. Some of these genes express inflammatory signal proteins, which affect cells in the immune system that are also involved in psoriasis. Some of these genes are also involved in other autoimmune diseases.
Topical treatments are creams, lotions, and ointments applied directly to the skin. These include moisturizers, prescription corticosteroids and vitamin D creams, and shampoos made with salicylic acid or coal tar. Topical treatments can effectively treat many types of mild to moderate psoriasis, but can be a little messy.
