If control becomes difficult or if psoriasis is generalized, the patient may benefit from phototherapy, systemic therapy and referral to a physician who specializes in the treatment of psoriasis. One of the most troubling features of topical corticosteroids is that patients develop tachyphylaxis, a phenomenon whereby medications that are highly effective initially, lose efficacy with prolonged use. To avoid tachyphylaxis and the other side effects of topical corticosteroids, regimens have been developed in which superpotent corticosteroids are applied twice daily for two weeks, after which they are applied on weekends only. Strong topical corticosteroids should also be avoided on the face and intertriginous sites, areas that are more prone to steroid side effects. The quantity of strong topical corticosteroids applied should be limited to 50 or 60 g per week, and occlusion should be avoided except on the scalp, palms, and soles.
