Quality of Life Studies directly comparing the effects of vitamin D analog monotherapy with very potent or potent steroid monotherapy consistently demonstrate the superiority of corticosteroids for the treatment of scalp psoriasis. A recent analysis of such trials revealed that corticosteroids provided approximately a 10% improvement in average scores compared with calcipotriol on a six-point global improvement scale (Investigators Global Assessment of Improvement).18 In addition, vitamin D derivatives take longer before an optimal improvement can be seen, approximately 8 weeks, as compared with only 2–3 weeks for corticosteroids.39 Given the efficacy of both corticosteroids and vitamin D analogs in the treatment of psoriasis, it is not surprising that combination products have been developed and have quickly become a favored medication in the treatment of scalp psoriasis.
Soaking in warm water with a bath oil, salt crystals or tar solution can soften the psoriasis and lift the scale. Bland soaps or soap substitutes are useful but detergents and antiseptics are not necessary and may irritate your skin. Excessively hot water should be avoided and the skin is best patted dry rather than rubbed. Since medication is absorbed better through damp skin, putting it on after a shower or a bath is helpful.

As a generalization, most oral medications act by targeting portions of the immune system. The only exception currently is a drug called acitretin (Soriatane), which is structurally similar to vitamin A. Since the immune system is necessary in order to survive, systemic treatments do have a downside. Drugs like methotrexate and cyclosporine are administered orally and can affect the liver, kidney, and bone marrow. A new oral medication recently approved for treatment of psoriasis is called Otezla® (apremilast). Otezla selectively targets molecules inside immune cells and adjusts (reduces) the processes of inflammation within the cell, which in turn helps treat psoriasis. This drug appears to be considerably safer that most of its predecessors but is also quite expensive.
