Methotrexate is prescribed in doses up to 30 mg per week for cutaneous psoriasis. Typically, a test dose of 2.5–7.5 mg is given one day in week one; if well tolerated, patients are titrated to 15 mg of methotrexate/week, given on one day per week. The dose is adjusted in accordance with the clinical response and side effects.
In terms of treatment response, as assessed by the PGA, steroids were more effective than vitamin D (RR 1.48; 95% CI 1.28 to 1.72; three studies, 1827 participants; NNTB = 5; 95% CI 5 to 7; moderate quality evidence). Statistically, the two-compound combination was better than steroid monotherapy, however the benefit was not clinically important (RR 1.13; 95% CI 1.06 to 1.20; two studies, 2226 participants; NNTB = 13; 95% CI 9 to 26; high quality evidence). The mechanism of action is unknown, although UVB light reduces DNA synthesis and can induce mild systemic immunosuppression. In PUVA, oral methoxypsoralen, a photosensitizer, is followed by exposure to long-wave UVA light (330 to 360 nm). PUVA has an antiproliferative effect and also helps to normalize keratinocyte differentiation. Doses of light are started low and increased as tolerated. Severe burns can result if the dose of drug or UVA is too high.

Although the disease typically improves during pregnancy and gets worse 4 to 6 weeks postpartum, the worsening of psoriasis in 23% of women during pregnancy has also been reported. Methotrexate and acitretin are contraindicated in pregnancy and are not recommended during breast-feeding. Cyclosporine may be considered as an alternative for use in pregnant women and should be avoided during breast-feeding due to the risk of immunosuppression of the baby. Regarding biotechnology drugs, recommendations are for discontinuation for varying times prior to conception, depending on the elimination half-life, due to the absence of controlled studies in pregnant women. However, information has been accumulating regarding its use, mainly in patients with inflammatory arthritis or inflammatory bowel disease. There are concerns on immunosuppression of fetuses exposed to biotechnology drugs at the end of the second and third trimesters of pregnancy, especially for monoclonal IgG antibodies. In addition, administration of live vaccines to newborns exposed to biotechnology drugs during the end of the second and third trimesters should be postponed until 6 to 7 months after birth. Breast-feeding is not recommended during therapy with biotechnology drugs, but may be considered reasonable in the future, as the amounts of drug found in milk are negligible. Decision of biotechnology therapy during pregnancy should be made on a case-by-case basis and shared with the patient.50
