Teledermatology Occasionally, seborrheic dermatitis of the scalp can be confused with scalp psoriasis since both can produce excess scale and can itch. Pathological examination of the skin biopsy material occasionally may be helpful in distinguishing the two entities. As a practical matter, this is rarely necessary because most patients with psoriasis have typical psoriatic lesions present in other areas, such as the elbows, knees, or torso. In addition, both conditions generally respond to similar topical medications.

In response to these chemical messages from dendritic cells and T cells, keratinocytes also secrete cytokines such as interleukin-1, interleukin-6, and tumor necrosis factor-α, which signal downstream inflammatory cells to arrive and stimulate additional inflammation.
PsoriasisSee What It Looks Like and How to Treat It There are many home remedies that can be used in the treatment of psoriasis. It is very important to keep the skin moist to avoid dryness. Petroleum jelly, shortening, or olive oil can be used as a moisturizer. Take fewer showers and baths to avoid stripping the skin of its natural oils. Adding salts, oil, or finely ground oatmeal to the bath can soothe the skin. Heliotherapy (medicinal sunbathing) can be effective in controlling psoriasis. There is also evidence that increased body mass is associated with psoriasis and that heavier individuals are more difficult to treat.

Although it appears on the skin, psoriasis is a disease of the immune system and is not caused or exacerbated by poor personal hygiene. People with psoriasis have a genetic tendency to develop the condition and no amount of cleaning of the skin can prevent psoriasis from precipitating or exacerbating. Stress, infection, skin injury, hormonal changes, excessive alcohol consumption, and exposure to certain medications can trigger a flare-up of psoriasis.

Tips for managing psoriasis There is no reason why your child’s education should be affected, but a visit to the school with your child to make sure their teacher knows the essential facts about psoriasis might help. Take leaflets and fact sheets with you to leave behind. Make sure your child feels comfortable talking about psoriasis with you and the school, so they will bring any problems to your attention. Ask the school how they will tackle any bullying issues if your child’s psoriasis is quite noticeable and devise avoidance tactics to pre-empt any possible situations occurring.

Oral medications include methotrexate (Trexall), acitretin (Soriatane), cyclosporine (Neoral), apremilast (Otezla), and others. Oral prednisone (corticosteroid) is generally not used in psoriasis and may cause a disease flare-up if administered.

Excimer lasers or pulsed dye lasers are used in laser therapy. A pulsed dye laser will create a concentrated beam of yellow light. When this light hits the skin, it converts to heat. The heat then destroys the extra blood vessels in the skin that contribute to psoriasis. Excimer lasers will deliver ultraviolet light to localized areas of the skin that help treat psoriasis. These lasers produce UV light in wavelengths similar to narrow-band UVB. Laser therapy uses intense doses of laser light to help control areas of mild to moderate psoriasis without damaging surrounding healthy skin. These can be quite effective for small plaques of psoriasis, but since only small areas of skin can be treated at once, they are not practical for extensive disease.
