Psoralens make the skin more sensitive to long-wave UVA (320-400 nm). Methoxsalen (Oxsoralen) is a psoralen that is taken by mouth before UV-A light therapy. More than 85% of patients report relief of disease symptoms with 20-30 treatments. Therapy is usually given two to three times per week on an outpatient basis, with maintenance treatments every two to four weeks to maintain remission. Adverse effects of PUVA therapy include nausea, itching, and burning. Long-term complications include increased risks of sensitivity to the sun, sunburn, skin cancer, skin aging and cataracts. Protective glasses must be worn during and after treatment to prevent cataracts. PUVA therapy is not used for children younger than 12 years of age.

Psoriasis is a skin condition underpinned by dysfunction of the immune system. It occurs in phases of relapse, remission (no symptoms) and exacerbation (severe symptoms). The primary symptom is papulosquamous skin rash, which is a red, scaly rash. Elbows, knees, scalp and trunk are the most commonly affected body parts; however, the disease can affect any body part. In mild cases the rash covers < 2% of the person’s body surface area. Disease is typically considered severe if it affects more than 10% of skin surface area. Up to 20% of people who have psoriasis have extensive skin rash. Joints are often also affected. The skin rash and other symptoms (e.g. those affecting the joints) which psoriasis causes are uncomfortable and may cause itching and burning sensations They also cause embarrassment which can have a serious impact on a person’s wellbeing.
When to seek medical advice Methotrexate is prescribed in doses up to 30 mg per week for cutaneous psoriasis. Typically, a test dose of 2.5–7.5 mg is given one day in week one; if well tolerated, patients are titrated to 15 mg of methotrexate/week, given on one day per week. The dose is adjusted in accordance with the clinical response and side effects.
