Acitretin is prescribed in doses of 10–50 mg daily. At doses of 10–25 mg in combination with UVB or PUVA phototherapy, acitretin is highly effective in clearing the condition in most patients with minimal side effects.31 Higher doses are associated with more adverse reactions but limited improvement in psoriasis. With chronic use, acitretin can be associated with the development of calcification of ligaments and tendons. Although these developments are usually asymptomatic, pain near ligaments and tendons in patients on acitretin warrants radiological examination to determine if the drug should be discontinued. Retinoids can benefit psoriatic arthritis in some patients.32

A 2017 meta-analysis found that ixekizumab, secukinumab, brodalumab, guselkumab, certolizumab, and ustekinumab were the most effective biologics for treating psoriasis. In general, anti-IL17, anti-IL12/23, anti-IL23, and anti-TNF alpha biologics were found to be more effective than traditional systemic treatments.

There are many topical and systemic treatments for psoriasis, but it must be born in mind that although many of them are effective in improving the appearance of the skin disease, none of them cure the condition.

Zollner TM SOURCES: Psoriasis often has a typical appearance that a primary care doctor can recognize, but it can be confused with other skin diseases (like eczema), so a dermatologist (skin doctor) is often the best doctor to diagnose it. The treatment of psoriasis usually depends on how much skin is affected, how bad the disease is (e.g., having many or painful skin patches), or the location (especially the face). Treatments range from creams and ointments applied to the affected areas to ultraviolet light therapy to drugs (such as methotrexate). Many people who have psoriasis also have serious health conditions such as diabetes, heart disease, and depression.
If these aren't effective, or your condition is more severe, a treatment called phototherapy may be used. Phototherapy involves exposing your skin to certain types of ultraviolet light.

UVA light penetrates deeper into the skin than does UVB light, and psoralen makes the skin more responsive to UVA exposure. This more aggressive treatment consistently improves skin and is often used for more-severe cases of psoriasis. PUVA involves two or three treatments a week for a prescribed number of weeks. UVA is often administered in a stand-up booth at a medical clinic or office.
