["sor-eye-ah-sus"] Bomb blast: bereavement due to (in ‘Bereavement due to traumatic death’) Science & Research Journal of Food Technology and Preservation It's pretty safe to say that scalp problems of any nature (itchiness, flaking, and the like), are highly annoying to anyone suffering from them. But scalp psoriasis — a common condition characterized by thick, red scaly patches of skin that can cover the entire scalp and be chronic — is arguably the most painful and inconvenient of them all. Scalp psoriasis treatment often comes in the form of oils, steroid solutions, and even ointments, but basic care for it starts in the shower with the shampoo you use. Those battling scalp psoriasis want to aim for a formula that simultaneously sloughs away flakes as it soothes irritation. New York City-based dermatologist and founder of Smarter Skin Dermatology, Sejal Shah, says that you should look for a shampoo comprised of ingredients such as coal tar, which is a dark liquid that's a by-product of the production of coke and coal gas from coal and is used in medications to treat psoriasis and seborrheic dermatitis. It may sound scary, but dermatologists say it significantly helps to slow down the rapid growth of skin cells while reducing inflammation. Apremilast, an oral PDE4 inhibitor, has been studied in three Phase 3 studies. Two pivotal trials, ESTEEM 1 and ESTEEM 2, have reported significant improvement in scalp psoriasis.47 The studies included patients with moderate-to-very severe scalp psoriasis (ScPGA ≥3) at baseline. The study design included a 16-week placebo-controlled period in which patients were randomized to apremilast 30 mg twice daily or placebo. At 16 weeks, the percentage of patients who achieved an ScPGA score of 0 (clear) or 1 (minimal) was significantly greater in the apremilast group (46.5% versus 17.5% in ESTEEM 1; 40.9% versus 17.2% in ESTEEM 2; P<0.0001 for both).47 During the open-label period, all patients received apremilast 30 mg twice daily from week 16 to week 32. In those patients who received apremilast from baseline, 37.4% and 32.4% of patients achieved an ScPGA score of 0 or 1 (ESTEEM 1 and ESTEEM 2, respectively). Of the patients initially randomized to placebo who switched to apremilast at week 16, 43.6% and 50.7% achieved an improvement in ScPGA score to 0 or 1 at week 32 (ESTEEM 1 and ESTEEM 2, respectively). By the end of week 52, in all patients who were PASI responders, an ScPGA of 0 or 1 was maintained in 83.3% and 62.5% of patients receiving apremilast (ESTEEM 1 and ESTEEM 2, respectively) and in 64.1% and 53.3% of those initially started on placebo and receiving apremilast in the open-label period only (ESTEEM 1 and ESTEEM 2, respectively).47 2. Areas of psoriasis may resemble dandruff, but the scalp will also have a silvery sheen and dry scales. Light therapy may be used if topical treatments are not effective. Natural sunlight or ultraviolet light (UVA or UVB) can be used to help clear the skin of psoriasis lesions. Light therapy may be used alone or in combination with medications. Ultraviolet light lessens the growth of plaques, redness, swelling and itching. Methoxsalen (Oxsoralen-Ultra) can be used with UV light also, called PUVA, and is effective in treating larger areas of widespread psoriasis. Laser light (UVB) is also used and can directly target psoriasis and avoid the surrounding skin. Psoriasis vulgaris L40.0 topical retinoids 41. Brezinski EA, Armstrong AW. For truncal psoriasis, it has been demonstrated that fluence is the single most important determinant in clinical clearing.5 The number of treatments, on the other hand, is not as important. Also, for trunk psoriasis, different plaque characteristics can be somewhat predictive of response. Thicker, scalier plaques tend to respond more slowly. This finding parallels what we noted in the present case. Our patient, who had extremely thick scaly plaques, required 23 treatments before clearing was noted.
