There are a variety of drugs administered systemically that are useful in controlling psoriasis. As a generalization, most oral medications act by targeting portions of the immune system. The only exception currently is a drug called acitretin (Soriatane), which is structurally similar to vitamin A. Since the immune system is necessary in order to survive, systemic treatments do have a downside. Drugs like methotrexate and cyclosporine are administered orally and can affect the liver, kidney, and bone marrow. A new oral medication recently approved for treatment of psoriasis is called Otezla® (apremilast). Otezla selectively targets molecules inside immune cells and adjusts (reduces) the processes of inflammation within the cell, which in turn helps treat psoriasis. This drug appears to be considerably safer that most of its predecessors but is also quite expensive.

Cold weather can make the skin really sore and so anything against the skin may irritate and cause pain. Perhaps slightly bigger shoes for periods when their feet hurt may allow some comfort. In the summer the heat may make their feet sweat, this again can cause some discomfort, so perhaps keeping them as dry as possible with some talcum powder may prove useful.
The treatment for psoriasis has changed from the previous gradual step-by-step approach. Today, doctors seek to optimize treatment from the first visit—whether with phototherapy or systemic therapies—based on the specific needs of each patient.

It currently affects roughly 7.5 million to 8.5 million people in the U.S. It is seen worldwide in about 125 million people. [35, 37] As with PUVA, the guidelines also recommend treatment with combinations of UVB and topical or systemic agents.  However, a study by Keaney and Kirsner gives objective reasoning for the benefit of narrowband UV therapy by showing decreases in T cells, dendritic cells, and interleukins within responsive psoriatic plaques compared with plaques that did not respond to therapy.  UVB also has the advantage of not leaving the patient with a prolonged period of photosensitivity as PUVA does.
