European guidelines for biologic treatment recommend that complete blood count and liver and renal function be evaluated at baseline, at months 1 and 3 of treatment, and then every 3 to 6 months thereafter while on the biologic agent.7 These recommendations are more stringent than those indicated in regulatory labeling and, based on the continual accumulation of data regarding the safety of these agents, some investigators have argued that laboratory testing might not be necessary at all.15

Non-steroidal anti-inflammatory drugs (NSAIDs) How do animals protect their skin Drugs by Class Psoriasis is characterized by an abnormally excessive and rapid growth of the epidermal layer of the skin. Abnormal production of skin cells (especially during wound repair) and an overabundance of skin cells result from the sequence of pathological events in psoriasis. Skin cells are replaced every 3–5 days in psoriasis rather than the usual 28–30 days. These changes are believed to stem from the premature maturation of keratinocytes induced by an inflammatory cascade in the dermis involving dendritic cells, macrophages, and T cells (three subtypes of white blood cells). These immune cells move from the dermis to the epidermis and secrete inflammatory chemical signals (cytokines) such as interleukin-36γ, tumor necrosis factor-α, interleukin-1β, interleukin-6, and interleukin-22. These secreted inflammatory signals are believed to stimulate keratinocytes to proliferate. One hypothesis is that psoriasis involves a defect in regulatory T cells, and in the regulatory cytokine interleukin-10.

Surgical Pearls For moderate to severe psoriasis that involves much larger areas of the body (>10% or more of the total skin surface), topical products may not be effective or practical to apply. This may require ultraviolet light treatments or systemic (total body treatments such as pills or injections) medicines. Internal medications usually have greater risks. Because topical therapy has no effect on psoriatic arthritis, systemic medications are generally required to stop the progression to permanent joint destruction.

Abortion for fetal abnormality Recently, the role of Th17 cells has been recognized in the proximal regulation of psoriatic cutaneous inflammation.9-10 IL-17A, the main effector cytokine produced by Th17 cells, stimulates keratinocytes to produce inflammatory mediators, establishing itself as a cytokine for binding between the innate and acquired pathways of the immune system. The present pathophysiological model of psoriasis establishes the potential for therapeutic intervention through the inhibition of the IL-17 pathway, with several monoclonal antibodies directed to this pathway, such as the recently approved secukinumab and the ones in development, ixekizumab and brodalumab, which represent a new therapeutic approach for moderate to severe psoriasis.13-14
