There is plenty of evidence that extensive psoriasis can have a very significant negative effect on a patient's self-image and emotions. This is especially true in social situations, although all aspects of life can be disturbed. Inverse psoriasis, which affects the genital skin, and scalp psoriasis can be particularly troubling. Psoriasis affecting the hands may make it impossible to interact normally with others. It is important to remember that there are ways to manage and treat psoriasis flares. It may seem as if one's quality of life has diminished, but there are many organizations that offer support to psoriasis patients. The National Psoriasis Foundation is an excellent source of accurate information as well as emotional support for afflicted patients.
Health Care Delivery Models Although psoriasis is just a minor irritation for some people, it can have a significant impact on quality of life for those more severely affected.
Narrow band UVB phototherapy. A newer type of psoriasis treatment, narrow band UVB phototherapy may be more effective than broadband UVB treatment. It's usually administered two or three times a week until the skin improves, and then maintenance may require only weekly sessions. Narrow band UVB phototherapy may cause more-severe and longer lasting burns, however.
Specifically, the epidermis is infiltrated by a large number of activated T cells, which appear to be capable of inducing keratinocyte proliferation. This is supported by histologic examination and immunohistochemical staining of psoriatic plaques revealing large populations of T cells within the psoriasis lesions. One report calculated that a patient with 20% body surface area affected with psoriasis lesions has around 8 billion blood circulating T cells compared with approximately 20 billion T cells located in the dermis and epidermis of psoriasis plaques.

Topical steroids are safe and relatively easy to use for plaque psoriasis, scalp psoriasis, flexural psoriasis, sebopsoriasis and psoriasis affecting the palms and soles. They are not very effective in nail psoriasis.

This article describes an algorithmic treatment approach for primary care physicians. The algorithm is based on treatment guidelines for psoriasis published by the American Academy of Dermatology,3 supplemented by a review of the medical literature. There currently are no evidence-based guidelines for the treatment of psoriasis. Because localized plaque psoriasis is the most common form, the algorithm focuses on the treatment of this type of psoriasis.
Disease is graded as mild, moderate, or severe based on the body surface area affected and how the lesions affect the patient's quality of life. To be considered mild, usually < 10% of the skin surface should be involved. There are many more complex scoring systems for disease severity (eg, the Psoriasis Area and Severity Index), but these systems are useful mainly in research protocols.

Mind & Brain Scalp psoriasis: Causes Because topical treatment regimens can be arduous and typically do not result in sustained clearance, patient expectations should be ascertained prior to initiation of therapy. Topical corticosteroids often can be used as monotherapy in patients with mild psoriasis.3 Topical vitamin D analogues and retinoids also can be effective; however, combined use of these agents with topical steroids should be considered to increase efficacy, and combination formulations can be prescribed to simplify application and improve adherence.
