Its symptoms, which usually emerge on the skin as flaky scales, are not only embarrassing but can also cause physical pain and intense itching. In addition, 10 to 30 per cent of psoriasis patients may develop psoriatic arthritis. Like other forms of inflammatory arthritis - such as rheumatoid arthritis - psoriatic arthritis causes joint pain, swelling, and stiffness. Reduce inflammation and itch There isn’t a cure for psoriasis, but there are good ways to keep the symptoms controlled. The epidermis is the uppermost portion of the skin. The epidermis is made up of layers of cells. The epidermis cells gradually change and move to the surface where they are continuously shed and replaced. Normally, a skin cell matures in twenty to thirty days during its passage to the surface where a constant invisible shedding of dead cells and scales takes place. Rise with us daily Multiple types of psoriasis are identified, with plaque-type psoriasis, also known as discoid psoriasis, being the most common type. Plaque psoriasis usually presents with plaques on the scalp, trunk, and limbs (see the image below). These plaques appear as focal, raised, inflamed, edematous lesions covered with silvery-white “micaceous” scales. (See Clinical Presentation.) About high blood pressure Excimer lasers or pulsed dye lasers are used in laser therapy. A pulsed dye laser will create a concentrated beam of yellow light. When this light hits the skin, it converts to heat. The heat then destroys the extra blood vessels in the skin that contribute to psoriasis. Excimer lasers will deliver ultraviolet light to localized areas of the skin that help treat psoriasis. These lasers produce UV light in wavelengths similar to narrow-band UVB. Laser therapy uses intense doses of laser light to help control areas of mild to moderate psoriasis without damaging surrounding healthy skin. These can be quite effective for small plaques of psoriasis, but since only small areas of skin can be treated at once, they are not practical for extensive disease. Also reported is a randomized, placebo-controlled trial assessing the safety and efficacy of etanercept for scalp psoriasis over a 24-week period.49 In this study, Group A received etanercept 50 mg twice weekly for 12 weeks, followed by etanercept 50 mg once weekly for 12 weeks (n=62). Group B received placebo twice weekly for 12 weeks followed by 12 weeks of etanercept 50 mg twice weekly (n=62). The primary end point was percentage change in PSSI at week 12.49 The etanercept group (Group A) had a mean PSSI improvement at week 12 of 86.8%, compared to only 20.4% in the placebo group (Group B) (P<0.0001). By week 24, Group A had a mean PSSI improvement of 90.6%, which was not significantly different from Group B, which had 79.1% improvement subsequent to initiating etanercept therapy at week 13. Eighty-six percent of patients in Group A achieved a PSSI-75 at week 12, compared with only 11% in Group B (P<0.0001). However, at week 24, the PSSI-75 rates were similar at 86% and 72% for Group A and B, respectively. There was also improvement noted in scalp surface area involvement, with a mean percentage improvement at week 12 in Group A of 84.6% versus only 14.9% in Group B. At week 24, Group A maintained this benefit at 84.8%, while Group B showed improvement at 75.4%. Changes in overall PASI improvement were consistent with the PSSI improvements and time to improvement was similar; treatment also provided a high level of patient satisfaction.49
