The presence of hair not only impacts the application and penetration of medications to affected areas, but also strongly influences treatment adherence. Patients commonly complain of the greasy effect of medications in this area, and difficulty removing products from their hair. Currently, topical medications are recommended as a first line of treatment in mild-to-moderate psoriasis but can also be used concomitantly with phototherapy, systemic or biological therapies for moderate-to-severe psoriasis. Topical corticosteroids, with or without the addition of the vitamin D analog calcipotriol, are the gold standard for the treatment of scalp psoriasis. Importantly, in recent years, there have been a number of new formulations introduced to the market (eg, foams, shampoos, and sprays) that enhance cosmetic acceptability and adherence. In severe or recalcitrant cases, systemic treatment should be considered.

France Phototherapy (light therapy): Ultraviolet (UVL) light, a portion of the solar spectrum with wavelengths between 290-400 nm, can have beneficial effects on psoriatic skin presumably by altering certain immune functions. Disease that is considered too extensive to be treated by topical approaches, that is usually greater than 5%-10% of the total body surface area, is an appropriate indication for this sort of treatment. Resistance to conventional topical treatment is another indication for light therapy. Although normal sunlight contains these wavelengths, self-exposure to sunlight must be done in under controlled conditions to minimize burns. In a physician's office, control of the amount of light energy administered to each patient is essential. Medical light sources use special wavelengths of light and timers to assure the correct dosage of light. Sunlamps and tanning booths are not acceptable substitutes for medical light sources. Ultraviolet light from any source is known to produce skin cancer, but this side effect is minimized when the light is appropriately administered in a physician's office.
In inverse psoriasis, genital lesions, especially in the groin and on the head of the penis, are common. Psoriasis in moist areas like the navel or the area between the buttocks (intergluteal folds) may look like flat red plaques without much scaling. This may be confused with other skin conditions like fungal infections, yeast infections, allergic rashes, or bacterial infections.

Irritation and staining can be avoided by washing off the anthralin 20 to 30 min after application. Using a liposome-encapsulated preparation may also avoid some disadvantages of anthralin.
