Acitretin is prescribed in doses of 10–50 mg daily. At doses of 10–25 mg in combination with UVB or PUVA phototherapy, acitretin is highly effective in clearing the condition in most patients with minimal side effects.31 Higher doses are associated with more adverse reactions but limited improvement in psoriasis. With chronic use, acitretin can be associated with the development of calcification of ligaments and tendons. Although these developments are usually asymptomatic, pain near ligaments and tendons in patients on acitretin warrants radiological examination to determine if the drug should be discontinued. Retinoids can benefit psoriatic arthritis in some patients.32

In such cases, removal of scale can help with the penetration of adjunctive medications.5 Due to its formulation, patients frequently report difficulty removing the product from their hair; this can be circumvented by applying a clarifying shampoo to the hair prior to entering the shower to aid in removal of the product.

The incidence of diabetes is 27% higher in people affected by psoriasis than in those without the condition. Severe psoriasis may be even more strongly associated with the development of diabetes than mild psoriasis. Younger people with psoriasis may also be at increased risk for developing diabetes. Individuals with psoriasis or psoriatic arthritis have a slightly higher risk of heart disease and heart attacks when compared to the general population. Cardiovascular disease risk appeared to be correlated with the severity of psoriasis and its duration. There is no strong evidence to suggest that psoriasis is associated with an increased risk of death from cardiovascular events. Methotrexate may provide a degree of protection for the heart.
Some people also notice that their psoriasis gets worse after eating certain foods, or as a result of allergies. Research has not yet conclusively proven that these factors can trigger psoriasis.

Plaque psoriasis is the most common form of psoriasis, affecting approximately 90% of patients. It is a chronic, inflammatory skin disorder in which there is an increase in the rate at which skin cells are produced and shed from the skin, appearing in the form of raised, red patches covered with a silvery scale (dead skin cells). The plaques can vary in number, size, and location but are often circular or oval shaped, and most frequently occur on the knees, elbows, scalp and sacrum (lower back). The plaques are often itchy and painful, and can crack and bleed.
