With full approval of our institutional review board, informed consent was obtained. The patient's scalp was mapped using clear plastic templates. Immediately before treatment, a score of 9 was noted on the modified Psoriasis Area and Severity Index (PASI), which is used to assess erythema, induration, and scaling (0, none; 1, slight; 2, moderate; 3, severe; and 4, very severe). Half of the large psoriatic plaque was treated with the 308-nm excimer laser using the scalp delivery device; the other half served as a control. Mineral oil was applied to both areas before the treatment began. A 2.5-cm spot size was used, with pulses delivered in a 25% overlapping fashion. The initial dose administered was 200 mJ/cm2, and subsequent doses were increased incrementally by 50 to 200 mJ/cm2 based on the patient's tolerance, ie, the sensation and/or clinical appearance of burning. Her final dose was 3600 mJ/cm2. The psoriatic plaque was to be treated 2 times per week, every 48 to 72 hours, for up to 30 treatments or until clinical clearing was noted (modified PASI score, ≤3). Our patient required only 22 treatments before clearing occurred. The average time of each painless treatment was less than 10 minutes. Between treatments and during the follow-up period, only the use of over-the-counter antidandruff shampoos was allowed.

A systematic review of 90 studies confirmed that patients with psoriasis had a higher risk of ischemic heart disease, stroke, and peripheral arterial disease but also a greater prevalence of risk factors for cardiovascular disease, compared with controls. The authors concluded that large prospective studies with long-term followup are required to determine whether psoriasis is an independent risk factor for vascular disease or is merely associated with known risk factors.  Another study identified psoriasis as an independent risk factor for cardiovascular disease in women, especially if they had psoriatic arthritis and suffered from psoriasis for a longer time period (>9 y). 
Excimer or pulsed-dye lasers can be used to treat psoriasis. The laser light is directed only at the affected areas of the skin. Excimer laser uses controlled UVB light whereas pulsed-dye lasers kill tiny blood vessels thought to contribute to psoriasis patches.

Does Scalp Psoriasis Embarrass You? You’re Not Alone The patient with refractory lesions may benefit from more advanced forms of treatment, such as phototherapy (ultraviolet B alone or psoralens plus ultraviolet A), outpatient treatment at a clinic specializing in psoriasis and systemic therapy with oral retinoids, methotrexate (Rheumatrex) or cyclosporine (Sandimmune). Combination therapy has also been shown to be effective, especially phototherapy in combination with topical anthralin, coal tar or calcipotriene (Table 4).
Take daily baths with warm water and gentle cleansers to help calm the skin and remove scaling. Avoid hot water; soak for at least 15 minutes. Although no formal study with the anti-TNF monoclonal antibody adalimumab in scalp psoriasis has been performed, a sub-analysis of the Phase 3 BELIEVE study was recently published.50 Of the patients enrolled, 663/730 (91.3%) had scalp involvement. The BELIEVE study compared adalimumab with or without Ca/BMD ointment (applied to body, not scalp) in a randomized, controlled safety and efficacy trial. Patients with baseline scalp psoriasis had more severe disease and were slower to respond initially compared to patients without, but this was no longer significant by week 8, at which point the majority of patients had achieved a PASI-75 response and continued to improve by week 16. PSSI responses correlated with PASI responses. The median PSSI score in patients at baseline was 14, at week 8 was 1, and at week 16 was 0. By week 8, 75.6% of patients had achieved a PSSI response (PSSI ≤4) and by week 16, a median decrease from baseline of 100% (mean 77.2%±96.9%).50
