The extent of skin involvement varies from a few patches in the majority of cases, to widespread and serious eruption. Cellulitis Multiple types of psoriasis are identified, with plaque-type psoriasis, also known as discoid psoriasis, being the most common type. Plaque psoriasis usually presents with plaques on the scalp, trunk, and limbs (see the image below). These plaques appear as focal, raised, inflamed, edematous lesions covered with silvery-white “micaceous” scales. (See Clinical Presentation.)

Like many other skin conditions, scalp psoriasis symptoms may come and go in cycles. Some people may not have symptoms for weeks, even months. You may notice that certain factors in your daily life may worsen (“trigger”) your scalp psoriasis symptoms to flare-up. Trigger factors can be different from one person to another. It is important to be aware of the factors and avoid them.

Psoriasis has a tendency to wax and wane with flares related to systemic or environmental factors, including life stress events and infection. It impacts quality of life and potentially long-term survival. There should be a higher clinical suspicion for depression in the patient with psoriasis. (See Prognosis.)

Sometimes, psoriasis patients may lose the condition naturally for long periods of time and completely on occasions. Careful planning and good compliance with treatment may leave the patient with minimal or no skin lesions for several years after completion of treatment.
Differential diagnoses Ultraviolet B (UVB) This is an effective choice and can be delivered as either broadband UVB (BB-UVB) or narrowband UVB (NB-UVB). UVB phototherapy is a good course of treatment for people with moderate to severe cases of psoriasis, patients with plaque psoriasis, those with thin plaques, and people who are generally responsive to natural sunlight.
Acitretin (Soriatane) is an oral drug used for certain types of psoriasis. It is not effective in all types of the disease. It may be used in males and females who are not pregnant and not planning to become pregnant for at least three years. The major side effects include dryness of skin and eyes and temporarily elevated levels of triglycerides and cholesterol (fatty substance) in the blood. Blood tests are generally required before starting this therapy and are needed periodically to monitor triglyceride levels. Patients should not become pregnant while on this drug and usually avoid becoming pregnant for at least three years after stopping this medication.
