To ensure that the full range of antipsoriatic treatments (ie, local therapy, UV therapy, classic systemic therapy, or biologicals) would be valid options, only patients with moderate to severe psoriasis according to the criteria of the Committee for Medicinal Products for Human Use29 were recruited. These included (1) those with a Psoriasis Area and Severity Index (PASI) of 10 or more30; (2) patients with psoriatic involvement of the hands, feet, or head; (3) patients with psoriatic arthritis according to Classification of Psoriatic Arthritis criteria31; and (4) those receiving systemic antipsoriatic therapy. Patients visiting our department for the first time, patients coming for follow-up visits, and those with and without ongoing antipsoriatic therapy were included. The study was approved by the Ethics Committee of the Medical Faculty Mannheim and performed according to the principles of the Declaration of Helsinki. ADHD Symptoms in Children? When used on the body, most vitamin D analogues were significantly more effective than placebo, with the standardised mean difference (SMD) ranging from -0.67 (95% CI -1.04 to -0.30; 1 study, 119 participants) for twice-daily becocalcidiol to SMD -1.66 (95% CI -2.66 to -0.67; 1 study, 11 participants) for once-daily paricalcitol. On a 6-point global improvement scale, these effects translate into 0.8 and 1.9 points, respectively. Most corticosteroids also performed better than placebo; potent corticosteroids (SMD -0.89; 95% CI -1.06 to -0.72; I² statistic = 65.1%; 14 studies, 2011 participants) had smaller benefits than very potent corticosteroids (SMD -1.56; 95% CI -1.87 to -1.26); I² statistic = 81.7%; 10 studies, 1264 participants). On a 6-point improvement scale, these benefits equate to 1.0 and 1.8 points, respectively. Dithranol, combined treatment with vitamin D/corticosteroid, and tazarotene all performed significantly better than placebo. What are less common forms of psoriasis? Two pivotal trials, ESTEEM 1 and ESTEEM 2, have reported significant improvement in scalp psoriasis.47 The studies included patients with moderate-to-very severe scalp psoriasis (ScPGA ≥3) at baseline. The study design included a 16-week placebo-controlled period in which patients were randomized to apremilast 30 mg twice daily or placebo. At 16 weeks, the percentage of patients who achieved an ScPGA score of 0 (clear) or 1 (minimal) was significantly greater in the apremilast group (46.5% versus 17.5% in ESTEEM 1; 40.9% versus 17.2% in ESTEEM 2; P<0.0001 for both).47 During the open-label period, all patients received apremilast 30 mg twice daily from week 16 to week 32. In those patients who received apremilast from baseline, 37.4% and 32.4% of patients achieved an ScPGA score of 0 or 1 (ESTEEM 1 and ESTEEM 2, respectively). Of the patients initially randomized to placebo who switched to apremilast at week 16, 43.6% and 50.7% achieved an improvement in ScPGA score to 0 or 1 at week 32 (ESTEEM 1 and ESTEEM 2, respectively). By the end of week 52, in all patients who were PASI responders, an ScPGA of 0 or 1 was maintained in 83.3% and 62.5% of patients receiving apremilast (ESTEEM 1 and ESTEEM 2, respectively) and in 64.1% and 53.3% of those initially started on placebo and receiving apremilast in the open-label period only (ESTEEM 1 and ESTEEM 2, respectively).47 Figures / Pustular Exp Dermatol. 2007; 16: 779-798 Insights in Gynecologic Oncology Open Access Journal Caring for the Critically Ill Patient Journal of Allergy & Therapy Open Access Journal EClinicalMedicine Svensson L Medical science has pointed out that the environmental factors contribute to stimulation or activation of certain cells in the immune system (psoriasis T-lymphocyte) which are drawn to the skin. These immune cells release certain chemicals (cytokines) which in turn activate the epidermal skin cells to multiply at a faster rate.
