Psoriasis is characterized by red, thickened plaques with a silvery scale. The lesions vary in size and degree of inflammation. Psoriasis is categorized as localized or generalized, based on the severity of the disease and its overall impact on the patient's quality of life and well-being. Patient education about the disease and the treatment options is important. Medical treatment for localized psoriasis begins with a combination of topical corticosteroids and coal tar or calcipotriene. For lesions that are difficult to control with initial therapy, anthralin or tazarotene may be tried. The primary goal of therapy is to maintain control of the lesions. Cure is seldom achieved. If control becomes difficult or if psoriasis is generalized, the patient may benefit from phototherapy, systemic therapy and referral to a physician who specializes in the treatment of psoriasis. However, since nails grow slow, it may take a while for improvements to be evident. Nail psoriasis can be treated with phototherapy, systemic therapy (medications that spread throughout the body), and steroids (cream or injection). If medications do not improve the condition of nail psoriasis, a doctor may surgically remove the nail. Psoriasis usually begins in early adulthood but it can start later in life. The rash can heal and come back throughout a person’s life. Psoriasis is not contagious and does not spread from person to person. In most people, the rash is limited to a few patches of skin. Natural sunlight or ultraviolet light (UVA or UVB) can be used to help clear the skin of psoriasis lesions. Light therapy may be used alone or in combination with medications. Ultraviolet light lessens the growth of plaques, redness, swelling and itching. Methoxsalen (Oxsoralen-Ultra) can be used with UV light also, called PUVA, and is effective in treating larger areas of widespread psoriasis. Laser light (UVB) is also used and can directly target psoriasis and avoid the surrounding skin. Many novel medications being researched target the Th17/IL-23 axis,[117] particularly IL-23p19 inhibitors, as IL-23p19 is present in increased concentrations in psoriasis skin lesions while contributing less to protection against opportunistic infections.[118] Other cytokines such as IL-17 and IL-22 also have been targets for inhibition as they play important roles in the pathogenesis of psoriasis.[118] Another avenue of research has focused on the use of vascular endothelial growth factor inhibitors to treat psoriasis.[58] Oral agents being investigated as alternatives to medications administered by injection include Janus kinase inhibitors, protein kinase C inhibitors, mitogen-activated protein kinase inhibitors, and phosphodiesterase 4 inhibitors, all of which have proven effective in various phase 2 and 3 clinical trials.[117][118] However, these agents have potentially severe side-effects due to their immunosuppressive mechanisms.[118] Rahman P Image courtesy of Hon Pak, M.D. and reprinted with permission from eMedicine.com, 2009 It's important to find someone, whether they have psoriasis or not, who will listen and let you express the strong feelings that accompany psoriasis. The mechanism of action of PUVA is unknown, but probably involves activation of psoralen by UVA light, which inhibits the abnormally rapid production of the cells in psoriatic skin. There are multiple mechanisms of action associated with PUVA, including effects on the skin's immune system. PUVA is associated with nausea, headache, fatigue, burning, and itching. Long-term treatment is associated with squamous cell carcinoma (but not with melanoma).[36][78] A combination therapy for moderate to severe psoriasis using PUVA plus acitretin resulted in benefit, but acitretin use has been associated with birth defects and liver damage.[79] Insurance Guide Guttate psoriasis most often affects people younger than 30.
