Pustular Advances in Dairy Research Open Access Journal Your Profile Tobin AM Ciclosporin, the most effective oral therapy for psoriasis, was fortuitously discovered to help psoriasis when it was administered to transplant patients with the disease. The side effects of ciclosporin are numerous and include hypertension, nephrotoxicity, hypomagnesaemia, hyperkalaemia, hyperuricaemia, elevation of liver function tests, development of paraesthesias, and hypertrichosis. In transplant patients, this immunosuppressive drug is associated with an increase in infections and lymphoproliferative diseases. If given for extended periods of time, nephrotoxicity occurs in most patients; hence, guidelines for the treatment of psoriasis with ciclosporin in the USA have suggested that it should not be administered for more than one year.33 All of the safety and efficacy data on the use of ciclosporin for psoriasis are based on doses up to 5 mg/kg daily, but the US Food and Drug Administration lists 4 mg/kg daily in the package insert. Nevertheless, it is usual to use higher doses for more severely affected patients.

No specific strategies prevent psoriasis, although healthy lifestyles that avoid obesity and reduced alcohol use can make control easier and increase the chances of at least temporary remission. Whenever possible, patients who are currently being treated for psoriasis or have a history of psoriasis should avoid over-the-counter and prescription medications known to exacerbate it. This includes the use of over-the-counter NSAIDs such as ibuprofen and naproxen.
That strategy has changed to a more patient-specific approach. Today, patients and their doctors can choose a treatment based on its effectiveness, the severity of their disease, lifestyle considerations, risk factors, and associated diseases (co-morbidities).

There were no significant changes in mean serum calcium, parathyroid hormone or urinary calcium/creatinine ratio.38

The exact cause remains unknown. A combination of elements, including genetic predisposition and environmental factors, are involved. It is common for psoriasis to be found in members of the same family. Defects in the immune system and the control of inflammation are thought to play major roles. Certain medications like beta-blockers have been linked to psoriasis. Despite research over the past 30 years, the "master switch" that turns on psoriasis is still a mystery.
