The rate of psoriasis varies according to age, region and ethnicity; a combination of environmental and genetic factors is thought to be responsible for these differences. It can occur at any age, although it most commonly appears for the first time between the ages of 15 and 25 years. Approximately one third of people with psoriasis report being diagnosed before age 20. Psoriasis affects both sexes equally.

Coal tar is available in multiple preparations, including shampoos, bath solutions, and creams. Coal tar may help reduce the appearance and decrease the flakes in psoriasis. The odor, staining, and overall messiness with coal tar may make it less desirable than other therapies. A major advantage with tar is lack of skin thinning.
MOTIVATION Fish oil or omega-3 fats Use: This is a newly developed oral drug for the treatment of psoriasis and psoriatic arthritis. Its efficacy is touted as being similar to the biologic response modifiers. Major side effects seem to be gastrointestinal so that it is recommended to start with a low dose and gradually increase it to the full therapeutic amount over about a week to avoid intolerable GI symptoms. No special laboratory tests for monitoring are required.

The stratum granulosum layer of the epidermis is often missing or significantly decreased in psoriatic lesions; the skin cells from the most superficial layer of skin are also abnormal as they never fully mature. Unlike their mature counterparts, these superficial cells keep their nucleus. Inflammatory infiltrates can typically be visualized on microscopy when examining skin tissue or joint tissue affected by psoriasis. Epidermal skin tissue affected by psoriatic inflammation often has many CD8+ T cells while a predominance of CD4+ T cells makes up the inflammatory infiltrates of the dermal layer of skin and the joints.

Psoriasis is an inflammatory disease that manifests most commonly as well-circumscribed, erythematous papules and plaques covered with silvery scales. Multiple factors contribute, including genetics. Common triggers include trauma, infection, and certain drugs. Symptoms are usually minimal, but mild to severe itching may occur. Cosmetic implications may be major. Some people develop severe disease with painful arthritis. Diagnosis is based on appearance and distribution of lesions. Treatment can include topical treatments (eg, emollients, vitamin D analogs, retinoids, coal tar, anthralin, corticosteroids), phototherapy, and, when severe, systemic drugs (eg, methotrexate, oral retinoids, cyclosporine, immunomodulatory agents [biologics]).
The most important consideration in treating scalp psoriasis is getting an effective medication into the skin. Both the hair and any scale covering the disease act as an impediment to treatment. Removal of the scale in a nontraumatic fashion is very important. This can be accomplished by shampooing frequently. Using tar, selenium, or salicylic acid-containing shampoos can be helpful. This may have to be done at least twice a day initially if the scale is sufficiently thick. Gently rubbing off the scale with the fingertips and not the nails is important.
