M Skin care tips The severity of psoriasis is decided by how much of the body it affects. Mild psoriasis affects less than 3 percent of the body, while moderate psoriasis affects between 3 and 10 percent of the body. Severe psoriasis is when more than 10 percent of the body is covered.
Overactive T cells also trigger increased production of healthy skin cells, more T cells and other white blood cells, especially neutrophils. These travel into the skin causing redness and sometimes pus in pustular lesions. Dilated blood vessels in psoriasis-affected areas create warmth and redness in the skin lesions.

Should I treat my psoriasis? A screening tool for PsA called the ‘Psoriasis Epidemiology Screening Tool’ (PEST) is available to help general practitioners and dermatologists identify patients for further evaluation by a rheumatologist. It is recommended that patients with psoriasis who do not have a diagnosis of PsA complete a PEST questionnaire annually and are referred on to a rheumatologist where necessary i.e. those patients scoring 3 or more out of 5 (please see diagram).

Biological factors Immunomodulatory agents (biologics—see Immunotherapeutics) include TNF-alpha inhibitors (etanercept, adalimumab, infliximab). TNF-alpha inhibitors lead to clearing of psoriasis, but their safety profile is still under study. Efalizumab is no longer available in the US due to increased risk of progressive multifocal leukoencephalopathy. Ustekinumab, a human monoclonal antibody that targets IL-12 and IL-23, can be used for moderate to severe psoriasis. IL-17 inhibitors (secukinumab and ixekizumab) are the most recently available biologics for moderate to severe psoriasis. Apremilast (inhibitor of phosphodiesterase 4) is the only available oral drug for psoriasis; however, early post-marketing data suggest it is not as effective as the TNF-alpha inhibitors.

Combination therapy may be helpful. Infliximab is given as a drip (infusion) into your vein at the hospital. You'll have 3 infusions in the first 6 weeks, then 1 infusion every 8 weeks. If there's no improvement in your psoriasis after 10 weeks, the treatment will be stopped.

Military Dermatology Several monoclonal antibodies target cytokines, the molecules that cells use to send inflammatory signals to each other. TNF-α is one of the main executor inflammatory cytokines. Four monoclonal antibodies (MAbs) (infliximab, adalimumab, golimumab, and certolizumab pegol) and one recombinant TNF-α decoy receptor, etanercept, have been developed to inhibit TNF-α signaling. Additional monoclonal antibodies, such as ixekizumab, have been developed against pro-inflammatory cytokines and inhibit the inflammatory pathway at a different point than the anti-TNF-α antibodies. IL-12 and IL-23 share a common domain, p40, which is the target of the recently FDA-approved ustekinumab. In 2017 the US FDA approved guselkumab for plaque psoriasis. There have been few studies of the efficacy of anti-TNF drugs for psoriasis in children. One randomized control study suggested that 12 weeks of etanercept treatment reduced the extent of psoriasis in children with no lasting adverse effects.

Patients with psoriasis should avoid injury to skin, including sunburn and other physical trauma, as these areas may develop psoriasis. The appearance of psoriatic lesions in previously uninvolved areas after irritation or trauma is known as the Köbner phenomenon. Patients with psoriasis should also, when feasible, avoid drugs known to worsen the problem (eg, chloroquine, beta-blockers, aspirin or other NSAIDs). They should also avoid alcohol to excess. An association has been made between nonalcoholic fatty liver disease and moderate-to-severe psoriasis. What is related to treatment and what is related to psoriasis itself is still being studied.
