Schaefer H Français When a person has two or more diseases at one time, these are called “comorbidities.” Right now there is no cure for psoriasis. The disease can go into remission where there are no symptoms or signs present. Current research is underway for better treatments and a possible cure. A 49-year-old white woman presented with a 20-year history of scalp psoriasis. Over the years, she had tried numerous topical treatments, such as 0.05% clobetasol propionate solution with 0.005% calcipotriene solution for 3 months, as well as various tar- and salicylic acid–based shampoos, with no success. She stated that the itching associated with her scalp psoriasis had worsened over the past year, often keeping her awake at night. She had abandoned medical treatment for 6 months. She was otherwise in good health and was not taking any medications. There was no family history of skin disease. Physical examination revealed thick, hyperkeratotic, scaly, erythematous, confluent plaques almost completely covering the scalp, including the postauricular area, clinically consistent with scalp psoriasis. Board Prep Plus The pathogenesis of this disease is not completely understood. Multiple theories exist regarding triggers of the disease process including an infectious episode, traumatic insult, and stressful life event. In many patients, no obvious trigger exists at all. However, once triggered, there appears to be substantial leukocyte recruitment to the dermis and epidermis resulting in the characteristic psoriatic plaques. Psoriasis is underpinned by a dysfunctional immune response, so biological agents target cells in the immune system including cytokines and T cells. These cells play a role in regulating the immune system’s response, including the dysfunctional (overactive) immune responses which cause psoriatic rash to develop. Targeting these cells modifies (reduces) the immune system response and reduces psoriatic skin rash. Geology & Earth science Generalised pustular psoriasis © Copyright State of Victoria 2018.
