Vitamin D-like compounds for psoriasis include calcipotriol, calcitriol, and tacalcitol. Oral retinoids were introduced for the treatment of psoriasis over the past two decades. Etretinate was the first retinoid available for psoriasis, but because of its long half-life, it could not be prescribed for women of childbearing potential due to the risk of long term teratogenicity. Acitretin, the active metabolite of etretinate, has a shorter half-life. Once it was approved, etretinate was withdrawn from the market. Acitretin is also associated with teratogenicity, but because of its shorter half-life, it can be given to women as long as they are not planning a pregnancy for three years after discontinuing its use. In the presence of alcohol, however, acitretin is converted back to etretinate, so caution must be used in women of childbearing potential to make sure they do not consume alcohol when taking acitretin.30

There are a variety of drugs administered systemically that are useful in controlling psoriasis. As a generalization, most oral medications act by targeting portions of the immune system. The only exception currently is a drug called acitretin (Soriatane), which is structurally similar to vitamin A. Since the immune system is necessary in order to survive, systemic treatments do have a downside. Drugs like methotrexate and cyclosporine are administered orally and can affect the liver, kidney, and bone marrow. A new oral medication recently approved for treatment of psoriasis is called Otezla® (apremilast). Otezla selectively targets molecules inside immune cells and adjusts (reduces) the processes of inflammation within the cell, which in turn helps treat psoriasis. This drug appears to be considerably safer that most of its predecessors but is also quite expensive.

D Psoriasis is characterized by an abnormally excessive and rapid growth of the epidermal layer of the skin. Abnormal production of skin cells (especially during wound repair) and an overabundance of skin cells result from the sequence of pathological events in psoriasis. Skin cells are replaced every 3–5 days in psoriasis rather than the usual 28–30 days. These changes are believed to stem from the premature maturation of keratinocytes induced by an inflammatory cascade in the dermis involving dendritic cells, macrophages, and T cells (three subtypes of white blood cells). These immune cells move from the dermis to the epidermis and secrete inflammatory chemical signals (cytokines) such as interleukin-36γ, tumor necrosis factor-α, interleukin-1β, interleukin-6, and interleukin-22. These secreted inflammatory signals are believed to stimulate keratinocytes to proliferate. One hypothesis is that psoriasis involves a defect in regulatory T cells, and in the regulatory cytokine interleukin-10.
